3 I. FUNDING OPPORTUNITY DESCRIPTION A. Program Description Applications for the Novel Presentation of Health Related Data (NPHRD) Program Announcement/Funding Opportunity are being solicited by the Assistant Secretary of Defense for Health Affairs, Defense Health Program (DHP). Through this Program Announcement/Funding Opportunity, the Joint Program Committee for Medical Training and Health Information Sciences (JPC-1) is soliciting applications/research proposals that will support the Department of Defense in developing novel presentations of health related data by leveraging the skills and talents of U.S. based businesses and academic research institutions. The Telemedicine and Advanced Technology Research Center (TATRC) is administering the application process for this funding opportunity, and the United States Army Medical Research Acquisition Activity (USAMRAA) is issuing this Program Announcement and will be negotiating all resulting awards. This Program Announcement/Funding Opportunity invites proposals from U.S. based businesses, academic research institutions, and multi-center collaborations, which may include international partners, to create novel representations of health data and generate new knowledge that supports healthcare improvements for DoD personnel and beneficiaries. Preliminary research proposals (i.e. pre-proposals) are required and will provide the basis for invited full proposals. Full proposals will be reviewed by an independent scientific peer review panel (PRP) and qualified projects will be selected for funding by a programmatic panel. The Government reserves the right to administratively reject proposals that are not consistent with the JPC-1 goals and priority areas identified below. The results of the research funded through JPC-1 are expected to contribute to improvements in healthcare delivery and/or management and to increase the body of knowledge available to professionals and practitioners in health, medical science, and related fields. The research impact is expected to benefit the military and civilian communities, in particular as it relates to the priority areas listed below. B. Priority Areas for FY2012 The expanded use of electronic medical records, personal health records, home monitoring devices, and other digital devices generate massive amounts of digital health related information. In order to use that information effectively, we need more innovative and novel ways to visualize the data. Clinicians and managers need to be better able to discern patterns in the data to make information contained in the data more useful for managing individual patients and for populations of patients. The wide range of data types and the diversity of users of the data add further complexity. The data is of all forms: text, numerical values, wave forms, audio, video and still images and the users of the data include patients, clinicians, administrators, and public health professionals. The solutions funded by this award are expected to demonstrate new and novel presentations of complex medical data to allow for insights into trends and discern patterns in the data. 4 Better methods for data visualization should improve the use of the data and make it more informative. There are opportunities for applying novel data visualization tools and techniques to clinical healthcare datasets. A few examples of novel and innovative visualizations of data sets are represented in the web sites below, although few have been applied to visualization of healthcare related data: Web Designer Depot ( published an article showing 50 examples of data visualizations and tools for creating visualizations. Web Designer Ledger published an article titled 15 Stunning examples of Data Visualization ( Smashing Magazine in 2007 published an article Data Visualization: Modern Approaches ( ). Visualizing.org Presentations at the Technology, Entertainment, Design (TED) conference and medical-focused TED conference (TEDMED) have focused on the visualization of large complex datasets. However, even in those conferences there has been limited attention paid to the visualization of large, complex health related data sets. Clinicians need a heads-up display of clinical information analogous to the airplane cockpit where a pilot can easily and quickly determine the status of his plane. For example, clinicians frequently annotate laboratory results as fishbone diagrams ( All physicians are trained using these and similar diagrams. They present a clear and consistent way to represent the results of laboratory results which allows clinicians to quickly assess the status of the lab result without sorting through information that may not be relevant. The use of these types of diagrams and other such visualizations of complex clinical data will make the information more accessible and useful. The Advanced Information Technology Group (AITG) in the Telemedicine and Advanced Technology Research Center (TATRC) has developed an Early Stage Platform (ESP) for Medical Training and Health Information Sciences research and development. The ESP is a computing and communications suite that contains an instance of the Armed Forces Health Longitudinal Technology Application (AHLTA) Clinical Data Repository (CDR) with clinical data representative of the data in the military health systems but which is not protected health information. The ESP provides an infrastructure for research initiatives. This infrastructure will be made available to awardees to develop and/or demonstrate novel visualizations of health data, but its use is not mandated. The following priority areas have been identified for FY 2012 funding by JPC-1. Applicants are required to align their proposals to one or both of the priority areas below and to elaborate in their proposals the target population for their solution and the benefits that it will bring. Prototype novel approaches to visualize complex healthcare data to clearly identify trends in population health data; the healthcare provider, healthcare administrator, and/or healthcare planner are the intended user of the prototype developed. 2. Prototype novel approaches to visualize healthcare data to clearly identify trends in individual patient s health records. Must consider the use of complex longitudinal data such as blood pressure, blood glucose, weight, etc.; the healthcare provider and/or patient are the intended user of the prototype created. The government is not interested in proposals that use standard displays of medical data that include basic bar, line, or pie charts. The government is not interested in a graphical user interface to access information stored in a database, but instead is interested in novel approaches to displaying and presenting longitudinal and other health data. The government is particularly interested in open source, non-proprietary technologies and proposals that result in publications and presentations that benefit the larger health IT community. C. Award Description USAMRMC is expecting to implement this research program through the award of cooperative agreements. No fee or profit is allowable under an assistance agreement. The supporting contracting office, USAMRAA, will negotiate and award assistance agreements against proposals selected for funding. More information on these funding instruments may be obtained from the USAMRAA Website at D. Eligibility Information 1. Principal Investigators must be independent investigators at any academic level from academia, research institutions, industry, and private foundations that possess the skills, knowledge, and resources necessary to carry out the proposed research. 2. Cost sharing/matching is not an eligibility requirement. 3. Refer to the Application Instructions, Appendix 1, for additional eligibility information. E. Funding: The amount of funding available for the Novel Presentation of Health Related Data for the first year is approximately $1.0 million; however, the government reserves the right to add additional funding. One to four awards are anticipated. 1. Funding of Applications received in response to this Program Announcement is contingent upon the availability of Federal funds for this program. 6 3. The maximum amount of funding for any award is $1.0 million in direct and indirect costs. Projects requiring lower levels of funding may also be submitted and are encouraged. 4. All direct and indirect costs of any subaward (subgrant or subcontract) must be included in the total costs of the primary award. 5. The applicant must submit a comprehensive budget that details the projected funding needed for the entire period of performance. 6. Strong justification must be provided to support the requested budget. Reasonableness of the budget, including total direct, indirect and other costs for the proposed research is a component of the peer review evaluation process. Refer to the Application Instructions, Section II.C.2, for budget regulations and instructions for the Research & Related Budget form. Direct costs may be requested for: 1. Salary 2. Equipment 3. Research supplies 4. Clinical research costs 5. Research related subject costs 6. Travel costs Travel costs to attend scientific/technical meetings for PI and associates. Costs shall not exceed $5,400 total per year. Travel costs associated with the execution of the proposed work. Reasonable costs for travel between collaborating organizations may be included and are not subject to the $5,400 total per year limitations. Travel outside the U.S., including between foreign countries, requires prior approval from the Grants Officer, unless identified in the awarded budget. Prior approval of the Grants officer must be requested 60 days before the proposed foreign travel. Recipient must include travel costs to attend one required In Process Review meeting per year. Costs shall not exceed $1,800 per year. 7. Other direct costs No fee or profit will be allowed under this Program Announcement. II. SUBMISSION INFORMATION The application process for this Funding Opportunity is being administered by the Telemedicine and Advanced Technology Research Center (TATRC). 7 Grants.gov ( by invitation only, and (3) submission of Applicant Response to Scientific Peer Review Summary Statement (if applicable) through the TATRC via Submission of the same research project to different funding opportunities within the same program and fiscal year is discouraged. The Government reserves the right to reject duplicative Applications. A. Where to Obtain the Application Package To obtain the complete full Application package, including all required forms, perform a Grants.gov ( basic search using the Funding Opportunity Number: W81XWH-12-NPHRD. B. Pre-Application (Pre-proposal) Submission Content, Form, Supporting Documentation and Screening All components must be submitted electronically through by the deadline on the title page. The Pre-application consists of the documents and forms found at Supporting Documentation (Quad Chart): In addition, Pre-applications are required to include a Quad Chart (see attached template) as supporting documentation. This data collection form is a PDF file that can be edited and saved using Adobe Acrobat Reader. Details on each section are provided below: Problem: Provide a bulleted summary of the problem addressed and its relation to the Project and Tasks described in the Program Announcement/Funding Opportunity, based on Section 4 of the pre-proposal. Proposed Solution: Provide a bulleted summary of the objectives of the work based on section 2 of the pre-proposal. Describe the plan for gaining access to clinical expertise and data. Picture: Insert a picture or other graphic that is representative of the work to be performed; this may for example show preliminary visualization work you have done, preview of what you are planning, or some other graphic that is representative of your proposal. Timeline and Cost: Identify at a high level the major planned activities or phases of the work and their duration on the chart provided, and provide the estimated budget by year. Pre-Application (Pre-proposal) Screening - All Pre Applications will be screened by the JPC for Medical Training and Health Information Sciences Program Panel. Invitation to submit Full Applications will result from this review. The review criteria for Pre Applications will be: Project Plans: Is the project plan/design sufficient to create a useful and novel representation of medical data that will address military and civilian health problems and be responsive to one of the priority areas for FY2012 funding in this Program Announcement? 8 Innovation: Is the design novel and innovative? Does the design offer new ways of visualizing medical data that is of relevance to military health system? Applicant and Key Personnel: How appropriate the applicant and key personnel are to conduct the proposed research. Scientific Rationale: Is there scientific rationale or literature that logically supports the proposed design and its feasibility? Clinical Access: Has the submitter demonstrated the ability to access clinical information and expertise? Estimated Budget: Whether the estimated budget is consistent with the funding limits for awards and appears consistent with the scope of work to be performed. Program Relevance: Did the PI adhere to the intent of the award mechanism? Existing Research Investments: All Pre Applications will be reviewed to ensure that there is no overlap with existing research investments Notification of Pre-Application Screening Results - Following the Pre-application screening, PIs will be notified of whether or not they are invited to submit an Application; however, they will not receive feedback (e.g., a critique of strengths and weaknesses) on their Pre-application. Pre-application notification dates are indicated on the title page of this Program Announcement. C. Full Application (Full Proposal) Submission Content and Form PIs will be notified if they are invited to submit a Full Application for an award under this Program Announcement. Full Applications will not be accepted unless the PI has received an invitation. 10 In the event that the DOD determines that such is required, a response template will be provided to applicants (for use in crafting replies) at the time of Summary Statement distribution to applicants. This will not be an opportunity to introduce new elements to the application, only clarify elements of the original submission. Every section of the response template must have a statement provided. All responses will be reviewed by the peer reviewers responsible for the original scientific review. Reviewers supplemental comments will be forwarded to Programmatic Review for consideration along with the original Summary Statements. Failure by applicants to submit the responses by the deadline identified in the notification of the requirement will result in administrative withdrawal of the application. E. Submission Dates and Times All submission dates and times are indicated on the title page of this Program Announcement/ Funding Opportunity. Pre-application (Pre-proposal) and Full Application (Full Proposal) submissions are required. Failure to meet any of the deadlines shall result in Application rejection. F. Other Submission Requirements Refer to the Application Instructions, Appendix 2, for detailed formatting guidelines. All Full Applications must be submitted through Grants.gov. Organizations are required to provide a Data Universal Number System (DUNS) number and register with the Central Contractor Registry (CCR) to submit Applications through the Grants.gov portal. Refer to the Application Instructions, Appendix 3, for information on Grants.gov requirements. If the proposal calls for study population, each PI is responsible for identifying and gaining approval to use a study population, to include military populations. The JPC for Medical Training and Health Information Sciences will not facilitate access to any study populations. However, the ESP s CDR will be made available to any Awardee that wants to use the data stored in it. If an extramural Applicant is proposing research involving access to service members, the Applicant is solely responsible for establishing a collaboration with a military partner to facilitate such access. III. REVIEW INFORMATION A. Full Application Review and Selection Process All Full Applications will be reviewed and evaluated using a two-tier review process. The first tier is a scientific peer review of Applications against established criteria for determining scientific merit. The second tier consists of a programmatic review by the JPC-1 for Medical Training and Health Information Sciences Panel of Applications against established criteria. The programmatic review compares submissions to each other and then recommends applications for DOD FY12 JPC1-Medical Training and Health Information Sciences (MTHIS)-NPHRD 10

11 funding based on scientific merit, military relevance, and the overall goals of the program based on programmatic criteria listed below in section III.B.2. The scientific peer and programmatic review processes are conducted confidentially to maintain the integrity of the merit-based selection process. Each level of review requires panelists to sign a non-disclosure statement attesting that Application and evaluation information will not be disclosed outside the panel. Violations of the non-disclosure statement can result in the dissolving of a panel(s) and other corrective actions. Organizational personnel and PIs are prohibited from contacting persons involved in the review process to gain protected evaluation information or to influence the evaluation process. Violations of these prohibitions will result in the administrative withdrawal of the organization s Application. Violations by panelists or PIs that compromise the confidentiality of the review process may also result in suspension or debarment of their employing organizations from Federal awards. Furthermore, the unauthorized disclosure of confidential information of one party to another third party is a crime in accordance with Title 18 United States Code The Government reserves the right to review all Applications based on one or more of the required attachments or supporting documentation. B. Full Application (Full Proposal) Review Criteria 1. Peer Review: All Applications will be evaluated according to the following scored criteria. The importance of each criterion is noted: Innovation (30%) How innovative is the project proposed and the proposed outcome? What evidence is there that the proposed project team can create innovative and novel representations of data? Project Plan (20%) How the scientific rationale and preliminary work, including critical review and analysis of the literature, support the proposed project and its feasibility and relevance. How the theoretical or conceptual framework from which the project is premised is defined. How the logistical aspects of the proposed study (e.g., communication plan, data transfer and management, and standardization of procedures) meet the needs of the proposed study. How well the PI acknowledges potential problems and addresses alternative approaches. How the plan for addressing unanticipated delays (e.g., delayed access to data) is likely to lead to success in completing the proposed study within the performance period. 12 Impact and Feasibility (20%) How the proposed project addresses health problems relevant to one of the funding opportunity priority areas. How the results of the proposed study will affect the magnitude and scope of potential applications related to the priority area. Budget (15%) How appropriate is the total budget for the proposed research? Personnel (10%) How the study team s background and expertise are appropriate to accomplish the proposed work (i.e., expertise in understanding clinical data, in visualization techniques, in creation of novel representations of data). How the levels of effort of the team are appropriate for successful conduct of the proposed project. Environment (5%) How the evidence indicates an appropriate environment and accessibility of institutional resources to support the project at each participating center (including collaborative arrangements). Evidence for appropriate organizational-level commitment from each participating organization/institution. 2. Programmatic Review: To determine the application s relevance to the mission of the DOD and JPC for Medical Training and Health Information Sciences, as well as to make funding recommendations, the following criteria are used by programmatic reviews: Responsiveness to Research Projects, Tasks, and Gaps How well the proposed project would meet the JPC for Medical Training and Health Information Sciences priority areas for FY2012 funding, if successful. How well the proposed study accelerates research efforts in the presentation and display of clinical information. Military Relevance How much the proposed project will contribute to accelerating the improved presentation of clinical data for military personnel to improve the quality of clinical care, if successful. Ratings and Evaluations of the Scientific Peer Reviewers Scientific merit of the proposed project will be considered in the context of the military relevance and programmatic review, and compared to all eligible applications under consideration. Application Review Dates All Application review dates and times are indicated on the title page of this Program Announcement/Funding Opportunity. E. Notification of Application Review Results Each PI and organization will receive notification of the funding recommendation. PIs will receive a scientific peer review summary statement on the strengths and weaknesses of the Application. IV. ADMINISTRATIVE ACTIONS After receipt of Pre-applications (Pre-proposals) from the TATRC or Full Applications (Full Proposals) from Grants.gov, the following administrative actions may occur: A. Rejection The following will result in administrative rejection of the Pre-application: Pre-application Narrative is missing. The following will result in administrative rejection of the Application: Project Narrative exceeds page limit Project Narrative is missing. Budget is missing. Submission of a Full Application (Full Proposal) for which a letter of invitation was not received. B. Modification Pages exceeding the specified limits will be removed prior to review for all documents other than the Project Narrative and Pre Narrative. Documents not requested will be removed. Following the Full Application (Full Proposal) deadline, you may be contacted by TATRC via with a request to provide certain missing supporting documents (excluding those listed in Section IV.A., Rejection). The missing documents must be provided by 5:00 p.m. ET on the second full business day following the date the was sent. Otherwise, the Application will be reviewed as submitted. Withdrawal The following may result in administrative withdrawal of the Pre-application or Full Application: Applicant Response to Scientific Peer Review Summary Statement was not submitted prior to specified deadline (if applicable). A Programmatic Panel member is found to be involved in the Pre-application or Full Application processes including, but not limited to, concept design, Application development, budget preparation, and the development of any supporting document. Submission of the same research project to different funding opportunities within the same program and fiscal year. The Application does not conform to this Program Announcement/Funding Opportunity description to an extent that precludes appropriate review. Total costs as shown on the Research and Related Budget form exceed the maximum allowed by this Program Announcement/Funding Opportunity. Inclusion of URLs with the exception of links to published references. Page size is larger than 8.5 inches x 11.0 inches (approximately cm x cm). D. Withhold Applications that appear to involve research misconduct will be administratively withheld from further consideration pending institutional investigation. The institution will be required to provide the findings of the investigation to the US Army Medical Research Acquisition Activity (USAMRAA) Contracting/Grants Officer for a determination of the final disposition of the Application. 15 D. Site Visits During the term of the award, the PI is encouraged to visit USAMRMC laboratories and institutes to discuss related work with USAMRMC scientists. All such visits must have prior funding and should be coordinated through the USAMRAA Contracting/Grants Officer Representative (COR/GOR) and the Contracting/Grants Officer. Funding for visits may be made available through the award instrument.
