1 2016 AAMC Clinical Care Innovation Challenge Pilot Awards Program Overview The AAMC Clinical Care Innovation Challenge (CCIC) Pilot Awards recognize the efforts of member institutions to advance care delivery. Pilot Award recipients are AAMC member institutions that have developed proposals for an innovative quality improvement (QI) program or project. Each award recipient will receive $10,000 to support the implementation and evaluation of the proposed QI program or project. Award recipients are expected to present their outcomes at the 2017 AAMC Annual Integrating Quality Meeting as well as complete other award related activities. Award Theme Areas With recent and emerging trends such as an aging and growing population, increasing reliance on technology, and exponential growth in medical discovery, there is a demand for rapid and simultaneous changes in medical education and health care delivery. This year s Challenge Award themes focus on optimizing the environment for learning, care and discovery, a priority of the AAMC Optimizing Graduate Medical Education (GME) initiative. Submissions should highlight QI programs or projects that implement change in the following areas to support learning, care, and discovery in GME: Patient safety, focusing on topics such as: o Techniques for patient engagement, including identification, notification, and education about participation in these programs o Training models that enhance population health and community-based training o Advance training in systems-based practice o Improvement of inter-professional, team-based training across disciplines and/or health professions Health care quality, focusing on topics such as: o Education and engagement of residents in organizational efforts to improve costs, quality, and patient safety (e.g. hospital acquired conditions, use of checklists) o Innovative use of telemedicine and other technologies for linking disparate sites of care and providers o Data analytics and tools for managing and leveraging clinical, financial, and quality data o Antibiotic resistance Care transitions, focusing on topics such as: o Enhance care coordination/communication across settings or providers o Strategies for engaging partners, such as post-acute care sites or community-based providers, in managing population health across settings o Population health-focused interventions to improve outcomes, care coordination, and/or quality/costs for designated populations o Strategies for integrating and educating residents and students about new payment models 2015 AAMC. 2 Submission Requirements Please review the application requirements below to ensure that this is the most appropriate track for your submission. Click on the associated application link at the end of this document to access the submission form. (Note: Proposals can only be submitted to one track. Multiple submissions of the same proposal will not be accepted.) An confirming the receipt of your application will be sent to the identified principal investigator within seven business days of receiving the application. Applications will be judged on their ability to demonstrate and include the following information about the proposed program or project: focus on at least one of the award themes the integration of at least one distinct professional group (faculty members, nurses, executive leaders) the incorporation of at least two academic mission areas (clinical care, education, and/or research) the need and importance of this program or project, both broadly and within your institution a description of the program or project highlighting the clearly defined objectives and goals, targeted population, implementation plan, desired outcomes, and its impact on the clinical learning environment details about what makes the program or project innovative in comparison to others identified in the literature identification of team collaboration as well as the interest and commitment of your institutional leadership in the success of this program or project explanation of the limitations and/or barriers and mitigating strategies used to address them the process for implementing the program or project a sustainability plan that highlights next steps and activities which would be supported by this award The following documents should be included as attachments in the Supplemental Materials section of the submission form: Statement of support from institutional leadership Curriculum vitae of the principal investigator and co-principal investigator (2 page limit for each) Institutional Review Board approval letter* Preliminary data and/or measurement* *If applicable. Review Process Submissions will be judged to determine the proposals that present the greatest impact and transformation to the clinical learning environment and care delivery. An Award panel comprised of nationally recognized executives (i.e. Chief Medical Officers, Chief Transformation/Strategy Officers) and patient safety and healthcare quality experts from AAMC member institutions as well as senior leadership from the AAMC will evaluate submissions and select the recipients. All decisions are final. Incomplete submissions will not be considered or reviewed. The checklist contains all required fields from the submission form. NOTE: Once you click on the online submission link, all responses to the form and supporting materials must be submitted during the same session. You will not be able to save your work and return. Identify and specify the limitations and/or barriers you expect to encounter while implementing your project, and explain potential mitigating strategies (limit 300 words). Describe your institution s interest and commitment in implementing this proposed program or project. Be sure to outline information, if available, such as preliminary data from a needs assessment, whether this proposed program/project aligns with an institutional or departmental strategic goal, and the potential benefits for your institution in implementing the proposed program or project.
