2 COHORT TWO Notification of Funding Availability (NOFA) Letter of Intent (LOI) Deadline: 11/29/2017 by 3:00 p.m. EST Background Healthcare Georgia Foundation s mission is to advance the health of all Georgians and to expand access to affordable, quality healthcare for underserved individuals and communities. Since inception, Healthcare Georgia Foundation has been committed to building effective nonprofit health organizations that have the capacity to develop, deliver and sustain high-impact health programs and services. The Foundation firmly believes that high-performing organizations delivering effective programs will achieve better health outcomes for clients, consumers, patients, and communities. This has been a priority among its strategic grantmaking investments throughout the Foundation s history. From , the Foundation implemented the General Operating Grant Program to strengthen and sustain an organization s capacity to fulfill its mission. Evaluation results from both the General Operating Grant Program and a previous capacity building project in partnership with Georgia Center for Nonprofits demonstrated that the Foundation should: 1) integrate an organizational capacity needs assessment tool to help nonprofits to determine their level of organization capacity prior to the grant request; 2) develop a resource toolkit to help nonprofits identify capacity building consultants; and 3) provide peer learning opportunities for grantees. Combined with current industry best practices and standards, the evaluation results led the Foundation to restructure the General Operating Grant Program in In 2016, the Foundation rebranded the General Operating Grant Program to EmpowerHealth, a twoyear pilot capacity building grant program to empower nonprofit health organizations to strengthen their leadership and to improve the ability for both staff and Board to operate at an optimal level to provide better results to communities. The Foundation partners with TCC Group to implement EmpowerHealth, and is using TCC Group s Core Capacity Assessment Tool (CCAT) throughout the application process. Capacity building is defined as the funding and technical assistance necessary to help nonprofits increase specific capabilities to deliver stronger programs, take risks, build connections, innovate and iterate. Supporting capacity building allows the Foundation to strengthen the way nonprofit health organizations provide services and collaborate so they are better able to sustain high-quality programs, achieve long-term performance and obtain better results. 1

3 In June 2017, the Foundation awarded grants to 11 organizations totaling $540,000 for Cohort One of EmpowerHealth. For more information about the Foundation s Cohort One grantees, please visit our website at This Notification of Funding Availability (NOFA) is to support Cohort Two of EmpowerHealth. Purpose and Goal Statement EmpowerHealth fits the Foundation s grantmaking priority area of Strengthening and Sustaining Health Nonprofit Organizations, Programs, and Workforce. The focus of this priority area is to address the critical organizational capacity needs that affect access, cost and quality of health and healthcare services. The Foundation recognizes that these structural/operational elements of nonprofit organizations weigh heavily on outcomes, yet are rarely the intended focus of philanthropic support. Therefore, the overall goals of EmpowerHealth align with the goals of the Strengthening and Sustaining Health Nonprofit Organizations, Programs, and Workforce priority areas which include: Enhance health nonprofit organizational effectiveness; Strengthen and sustain high-impact health programs, services, and supports; and Build/strengthen Georgia s health professions workforce capacity. Scope of Work The Foundation s EmpowerHealth capacity building program will award grants to eligible applicants for the purpose of strengthening and sustaining the organization s capacity to fulfill its mission. TCC Group s evidence-based organizational assessment instrument, the Core Capacity Assessment Tool (CCAT) will be used throughout the application process by nonprofits who desire to enhance their ability to accurately identify organizational areas that need strengthening. The Foundation will provide eligible organizations with an opportunity to complete the CCAT. Funded organizations will complete a capacity building project based on the organizational needs identified in the CCAT and will participate in peer learning workshops to share what they have EmpowerHealth Cohort TWO NOFA Issued November 1,

4 _ learned with colleagues, build local capacity, and establish a community of practice. This approach provides organizations with access to valuable tools and resources to guide them in determining priority needs for capacity building, builds peer-focused learning communities that lead to increased sustainability, improves collaboration among health nonprofits and improves health outcomes throughout Georgia. Completing the CCAT and participating in subsequent capacity building activities involves a full commitment from the organization s Board and staff. Evaluation Evaluation tools and resources can be found on the Foundation s Evaluation Resource Center website at Registration is free and provides full access to an evaluation toolkit, worksheets and case studies. Please Note: Post award, all organizations selected to receive grants will be required to submit a logic model and an evaluation plan listing evaluation indicators demonstrating increased organizational capacity for their project. Eligible Organizations Nonprofit health organizations, including nonprofit hospitals, that are exempt from Federal income tax under provisions of Section 501(c)(3) of the IRS code Quasi-governmental health entities Organizations must be located in Georgia Previous grantees must be in good standing with the Foundation (e.g. submitted progress and final reports and met grant objectives) Ineligible Organizations and Activities Colleges/universities, for profit organizations, and governmental agencies are ineligible to apply. This includes Institutes, academic centers, and other entities affiliated with a college and/or university. State and local public health departments EmpowerHealth Cohort TWO NOFA Issued November 1,

5 Any organization with an active grant awarded from Healthcare Georgia Foundation. If you are a current grantee and have a question about eligibility, please contact your program officer. The Foundation will not accept more than one LOI from an organization. Organizations requesting funds for Direct Services. The Foundation defines Direct Services as the delivery of a particular project, health service, program, or activity undertaken by a health nonprofit organization to impact an individual/population s health status and/or expand access to affordable, quality healthcare. The funding announcement for the Direct Services Grant Program will be announced in the summer of The Foundation does not make grants for activities that exclusively benefit the members of sectarian or religious organizations. Application Process and Review Criteria EmpowerHealth will have two phases and interested organizations must participate in all required activities in order to be eligible to move forward in the application process. The Foundation will host an EmpowerHealth Informational Webinar on November 9, 2017 at 2:00 p.m. Participating in the Informational Webinar is not required to submit a Letter of Intent (LOI), however the Foundation strongly encourages potential applicants to view the webinar. The purpose of the webinar will be to provide additional information about the EmpowerHealth program, required scope of work and the application requirements. Phase I: November 2017 March 2018 Submit a Letter of Intent by November 29, 2017 by 3:00 p.m. EST to the online portal (submission of a LOI is required to be eligible to participate in this funding program). The link to submit the Letter of Intent is available on the Foundation s website. All LOIs will be reviewed for eligibility. Applicants that are deemed ineligible will be unable to move forward in the application process; Attend one of three CCAT Orientation Targeted Capacity Building Workshops. These workshops serve as orientation webinars to the Core Capacity Assessment Toolkit (CCAT). The Foundation is hosting three webinars to accommodate organizations different schedules; the three CCAT orientation webinars will contain the same information. Interested organizations must participate in one CCAT orientation webinars to be eligible to complete the CCAT; Complete the online CCAT to identify areas within the organization that need strengthening; and Participate in one of two CCAT Group Interpretation webinars to learn how to interpret your CCAT results and prioritize the funding request. 4

6 The Foundation will close the LOI at 3:00 p.m. EST on November 29, Once the deadline has passed, you will not be able to edit, complete, save, or submit your LOI and the Foundation staff will not be able to access your application. Only organizations that submit the LOI will receive the information to participate in the CCAT orientation webinars, complete the CCAT, receive group and individual coaching, and apply for the full online application. Phase II: March 2018 December 2019 Submit a full online application to Healthcare Georgia Foundation based on the needs identified in the CCAT by April 13, 2018 by 3:00 p.m. EST; All applications will undergo a competitive and rigorous review and funding selection process; and Selected grantees will conduct capacity building activities and be required to participate in capacity building and peer learning workshops to share lessons learned and to develop a learning community. The EmpowerHealth full online application materials will be available on March 16, 2018 and will be due by April 13, 2018 by 3:00 p.m. EST. Applicants will be eligible to apply for up to $50,000 for a grant period of 18 months to address capacity building needs and priorities identified in the CCAT. In order to be eligible to submit a full online application, all interested organizations must have: 1) submitted a Letter of Intent; 2) attended one of three CCAT Orientation webinars; 3) completed the CCAT; and 4) attended one of two CCAT Group Interpretation webinars to interpret the CCAT results and prioritize organizational capacity building needs. Applicants that complete all required activities will be eligible to obtain individual organizational capacity building technical assistance from the TCC Group and/or an EmpowerHealth preferred Consultant to further interpret the CCAT results and prioritize capacity building needs. The Foundation anticipates that approximately 10 grants will be awarded. The number and amount of grant awards will be based on the number of applications received and the availability of Foundation funds. 5

The Foundation strongly encourages potential applicants to participate in this webinar. The webinar will provide additional information about the EmpowerHealth program, the scope of work and the application requirements. The webinar will be recorded and available on the Foundation s website following the event. Register for the webinar here: LOIs due by 3:00 p.m. EST (required) CCAT orientation webinars (The Foundation is hosting three webinars to accommodate organizations different schedules; the three CCAT orientation webinars will contain the same information). Interested organizations must participate in one CCAT orientation webinar to be eligible to complete the CCAT. The registration link will be sent via a confirmation to organizations that submit the LOI. If you submit a LOI, please hold one of the following dates/times on your calendars: Targeted Capacity Building Workshop (CCAT Orientation Webinar Dates and Times) December 5, 2017: 12:45 p.m. 1:45 p.m. EST December 13, 2017: 1:45 p.m. 2:45 p.m. EST December 15, 2017: 2:15 p.m. 3:15 p.m. EST December 18, January 26, 2018 Eligible organizations take the CCAT (required) 6

8 February 6 and 7, 2018 February 12-March 9, 2018 March 16, 2018 April 13, 2018 June 2018 July 2018 December 2019 TCC Group will conduct CCAT group interpretation webinars to help you to interpret your CCAT results. Both webinars are from 3:45 p.m. 4:45 p.m. EST. (required) TCC Group and/or an EmpowerHealth Preferred Consultant will provide individual coaching calls to organizations to help organizations interpret the CCAT results and prioritize grant request activities (Limited slots available) EmpowerHealth full online application released EmpowerHealth full online application deadline (required) Grant awards announced Grant period begins 18-month grant period ends Frequently Asked Questions Q: Who is eligible to apply? A: The applicant organization should be tax exempt under section 501(c)(3) of the Internal Revenue Code and defined as not a private foundation under Section 509(a). Nonprofit Hospitals and Quasi- Governmental Agencies are also eligible to apply. Q: I applied for the EmpowerHealth program last year, but was not awarded a grant. Is my organization eligible to apply? A: If you applied to the EmpowerHealth program last year and do not know why your application was declined, please contact Andrea Kellum, Program Officer, at akellum@healthcaregeorgia.org or If your organization was deemed eligible to apply for EmpowerHealth, you are eligible to reapply for Cohort Two as long as you meet the general eligibility criteria listed on page 3 of the NOFA. Organizations that completed all of the steps of the EmpowerHealth program last year, including submitting a full online application, will still be required to submit a Letter of Intent for Cohort Two. More information about what will be required for applicants that completed all the required activities in Cohort One will be provided during Informational Webinar on November 9th at 2:00 p.m. EST. Q: How do I know if my organization is considered a health organization and is eligible to apply? 7

9 A: Health organizations provide services and/or programs that deal with the diagnosis and treatment of disease, or the promotion, maintenance and restoration of health. Your organization must also be providing services or programs that fit the Foundation s mission to "advance the health of all Georgians and to expand access to affordable, quality healthcare for underserved individuals and communities." Q: What is a quasi-governmental entity? A: Quasi-governmental entities are supported by the government, but managed privately. A community mental service board is an example of a quasi-governmental organization. Q: My organization has a current grant with Healthcare Georgia Foundation? Can we apply? A: If your organization has an active grant with the Foundation, you are not eligible to submit apply and submit a Letter of Intent. If you are unsure whether your grant is active, please contact your assigned program officer. Q: I am a previous Foundation grantee, but I'm not sure if my organization is in good standing. A: Please contact the Foundation and ask for your program officer. Q: How do I access the Letter of Intent (LOI) I started? A: Use the following link once you have started and saved your LOI. This will give you full access to your account. Q: Can I submit more than one LOI? A: The Foundation will only accept one LOI per organization. Q: If I am in the middle of my LOI, can I save and continue to work on it later? A: Yes. Click the Save and Finish Later button located at the bottom of the page. This will save all your work. When you are ready to continue, click the returning user tab on the Foundation s website or click on the link ed to you when you created your account. Q: I have submitted a previous online application to the Foundation, but cannot remember my password. 10 Q: What happens after I submit my LOI? A: You will receive an confirmation. The confirmation will include the registration link to gain access to one of the three CCAT orientation webinars. If you do not receive an confirmation, check your junk or spam folders. If you still do not have the confirmation, contact Javier Sanchez. Q: My organization received an award for the Foundation s Two Georgias Initiative. Can I also apply for EmpowerHealth? A: If your organization was funded as the lead organization applying for the Foundation s Two Georgias Initiative, you are not eligible to apply for EmpowerHealth. Partner organizations not serving as the lead organization for The Two Georgias Initiative are eligible to apply as long as they meet the eligibility criteria defined on page 3 of this NOFA.
