A true copy of the Applicant s Birth Certificate. Two recent passport-size photographs with the full name of the candidate written on the back. (One photograph should be stapled (not glued or gummed) securely to this form in the space provided). Candidates must request two referees to complete their statements on the prescribed Supporting Letters and send them attached to this application. 1.3 The Form must be submitted via post to The Mercator Foundation, Post Office Box 121, Tung Chung, Lantau, Hong Kong. 1.4 The completed Form may also be submitted via to enquiries@mercatorworld.com Page 1

If awarded a Grant I undertake to observe all the conditions of the award for which I now apply.
