3 BACKGROUND & NEEDS Impact Statement IMPACT STATEMENT In our five years serving the Myakka City area, we have consistently added programs to assist the less fortunate in our area. Perhaps our largest undertaking is our monthly food pantry. Since opening the pantry in 2015, registration grows monthly, with current participating families around 175. In October, 2016, we were asked by migrant education to open an emergency food pantry for the migrant families who were returning to Florida from Michigan with very little food and money. This emergency pantry introduced the migrant community to our efforts, and registration from the migrant families continues to grow each month. With the growth, we have been able to increase our food purchases to accommodate the growing numbers. We are fortunate to have been given donated funds in 2016 to purchase freezer and refrigeration unit so we can offer frozen meats and fresh produce in addition to shelf-life food. Additionally, we have been able to purchase a used truck to transport food from the Food Bank of Manatee (approximately 45 minutes each way). The truck has been instrumental in aiding us with growth, but also requires extra expenses such as insurance and fuel costs. In late 2015, we began a backpack program at Myakka City Elementary, providing backpacks of food to those children on free/reduced lunches. 70% of the school qualifies, and in 2016/2017 school year, we are supplying 185 backpacks each week to hungry children. We have already secured funds for the 2017/2018 school year with a generous grant from Kiwanis Club of Bradenton, along with a matched grant from a private foundation. We continue to support those struggling financially due to significant medical issues in Myakka City. Since our incorporation in 2011, we have helped families with over $30,000 in financial assistance. Our Angel Tree program has exploded. In our third year 2016 we provided Christmas gifts to 85 families 308 children! Of these families, 69 were migrant farming families. We are very fortunate to have fantastic community support for this program, with gift donations valued around $4,000. In addition, a grant was received from Lakewood Ranch Community Foundation, donation was given by Mosaic Wingate employees, a toy drive was held by Buckaroo Roadhouse, and we had several companies adopt up to 25 children each. Needs Statement NEEDS STATEMENT Our most urgent need is a permanent building! We are currently renting space on a month-to-month basis, and could lose space immediately if the property sells. We have obtained a 1,600 square foot portable building from East County Fire Department. We are hoping to attempting to purchase property for placement of the portable. The property will cost around $100,000, and we are asking for owner financing with a balloon in 5 years. In addition, we will need to add septic, well, culvert, and parking, which may cost around $20,000. This is a necessity and high priority to ensure that our programs will continue. Our current rental space is too small to accommodate much more food pantry growth (only 900 square feet). Additionally, we cannot add programs such as AA meetings (currently none offered in Myakka City), ESL programs (requested due to large migrant populations), counseling, internet access, and so many other programs badly needed in the area. Property is crucial at this point. Without obtaining property on which to place our portable building, we may not be able to continue if we lose our rental space. Once property is secured, we will apply for a government grant to build a permanent building. We are fortunate to have access to the grant writer for Catholic Charities through the Diocese of Venice. She will assist us in securing the federal grant in the future, but for now, purchase of property is essential. Background Statement 3

4 Myakka City Foundation was incorporated in March 2011, and our name was changed to Giving Alliance of Myakka City, Inc. in February Our Board Members have all previously worked with major charities in our area to secure funds for their missions. In the process, we realized that our small, rural area was making substantial efforts for these charities. While an enormous effort was made, our community saw little return for our efforts. This is when we decided to form our own local charity, and Myakka City Foundation was formed! The heart in our community is overwhelming, and the community is crucial in our efforts - "Neighbors Helping Neighbors". Statement from the Board Chair After working with other national non-profits, our Board wanted to localize our efforts. We decided to form Giving Alliance of Myakka City, Inc. with our main intention to help our neighbors struggling financially due to medical hardships. Since then, we have expanded into many other areas such as providing food for the hungry in the area. Myakka City is a small, rural area. Often overlooked for larger, more prestigious areas, Myakka City is resourceful in helping our neighbors in the community. Since our inception, we have consistently grown and expanded our services to the Myakka City area. I am proud to serve on the Board with my fellow Directors. I look forward to our continued growth and success in the area. 5 PROGRAMS Financial Assistance/Individual Fundraisers Description Part of the mission of Myakka City Foundation is to assist those Myakka City residents who are struggling financially due to medical issues. To complete the application process, applicants must (a) provide Myakka City residency, (b) provide proof of medical condition, (c) provide financial disclosure, and (d) provide qualified payments requested. We hope to help as many people as we can who fit our criteria, but it is all dependent upon our fundraising efforts. We have had several of our applicants tell us that they were referred to us by pastors, neighbors, friends or family. We are pleased to have supporters who recognize our efforts to help our neighbors. The program is constantly monitored and changed as required. We have a stringent application process to ensure that only qualified Myakka City residents are given financial assistance. Our success is monitored by how many people we are able to assist. We evaluate our application process to determine how the program needs to be changed to most effectively give assistance to as many of our neighbors in need as possible. In response to this monitoring process, we noticed that many applicants needed help in filling out the forms. We identified a retired nurse who is now volunteering as our "Assistance Coordinator" to help applicants properly fill out the forms. A call was made to us from a mother whose teenage son needed surgery for a hernia. She did not have the cash for the operation. By expediting the application process, we were able to financially assist her, and the son received surgery within the week. In another instance, a local hairdresser died suddenly of a heart attack at the age of 60. Her family had no funds for cremation. We were able to expedite the application process to assist the family so they could honor their loved one with respect. We also financially assisted a "hero" who suffered massive burns while saving a child from a burning vehicle. 5

6 Food Pantry Description Implemented in 2015, we currently operate a monthly food pantry, supplying food to hundreds of low-income people in the area. In the months of October and vember, Walgreens also participates at our food pantry to supply free flu shots. In addition to USDA Commodity Foods from Food Bank of Manatee, we also purchase additional shelf-life foods, frozen meats, fresh produce, and breads/pastries to offer our participants. Local residents donate eggs and handmade soaps each month. At one of our food pantries, a 10-year-old child asked to speak to one of the managers of the backpack program. He wanted to personally let us know how much he loved the program and thank us for making it possible. This child made the day special for all of us! Our numbers continue to grow each month - a great indicator of success. We receive much gratitude from the people we serve. Feedback regarding the food pantry and backpack programs is uplifting and motivates us to do more. These food programs were much-needed in our rural area. Before we opened our food pantry, many people had to travel into Lakewood Ranch (Woodland Baptist Food Pantry is nearest) - approximately a 20 minute drive. A number of local churches refer the needy of their congregations to us. Program success is evaluated by the number of people we feed, and the nutrition of the foods we offer. With a generous donation in 2016, we added freezer and refrigeration to be able to offer frozen meats and fresh produce. Positive feedback from participants is heartwarming. We currently have approximately 175 families registered for our food pantry - that's about 750 people we feed each month. We received a tearful call from a Myakka City widow who said that she had become hurt and couldn't work. She had depleted her food, and had no funds to fall back on. We opened the food pantry the next day just for her. She cried the whole time she was there - so happy that she had a local resource to help her through this tough time. Since that day, she has been able to get back to work and provide for herself again. It was really special to be there for her when she needed us most! We ALWAYS open the pantry at the emergency request of a hungry person. 6

7 "Angel Tree" and Christmas Magic in Myakka Description Each year, we identify needy families in Myakka City. We put up Christmas trees around town with gifts wished for by those needy children. We ask the community to donate gifts, and the community donates more than $4,000 worth of those gifts! Giving Alliance of Myakka City provides those gifts not provided by the public to help make Christmas brighter for those who have less. (Partial funds were given through a grant by Manatee Community Foundation - thank you!) Buckaroo Roadhouse hosts a toy drive, and donations are given by the employees of Mosaic's Wingate Mine, Postnet, Community Church of Lakewood Ranch, Myakka City Grill, and other local companies. We also host a Christmas Party "Christmas Magic in Myakka" for ALL residents. Party includes food, door prizes, DJ, music, and Santa Claus. Both of these events have been highly successful - with Angel Tree program serving 85 families (308 kids) and the Christmas program hosting 200 kids. Serving 308 kids from 85 families with gifts for Christmas was the highlight of The smiles and happiness are a tribute to the success of the program. Additionally, the generosity of the community (over $4,000 worth of gifts donated) is humbling. We have an AMAZING community! 2016's Angel Tree program almost doubled over In our current location, we will be strapped to accommodate growth in 2017 for this program. Space of our own in a larger building is vital to allow this program to continue to grow. We do not want any child to be disappointed at Christmastime, so we will do what it takes to be sure that each child gets at least one gift. With larger space - as we desperately are trying to secure - we will be able to continue to grow this program and reach more needy families at Christmastime. Success of this program is determined by how many families we are able to help with Christmas gifts, and how many people attend the Christmas parties. Flyers are translated into Spanish and distributed among migrant families as well as full-time residents of Myakka City. This year's Christmas party attendance was down, and our Board has determined that the year 2017 will combine the Angel Tree gifts with the Christmas party rather than doing them separately. This will not only cut our costs and volunteer time, but will allow us to more accurately plan for the number in attendance. 7

8 Program Success Examples Seeing smiles on children's faces are the best examples of success! During the pickup period for the families who made requests, many of the parents were tearful and overwhelmed by the generosity of the community. I was told by several people that their children would not have had much under the tree (if at all) if it weren't for our efforts. Christmas has never been so special for those of us who volunteered for this program! 8

9 Backpack Program Description Our Backpack Program (providing weekend food for students on free/reduced lunches) began at Myakka City Elementary school for the 2015/2016 school year. For the 2016/2017 (existing) school year, a private foundation donated $14,000 to fully fund the program using Blessings in A Backpack. Use of the program ensures consistent, nutritious food each week. We currently serve all Myakka City Elementary students who are on free/reduced lunches - that's 185 students each week. For the school year 2017/2018, the same private foundation has offered up to $7,000 toward the program - so long as we can get matching funds. We are extremely fortunate to have been given a grant by the Kiwanis Club of Bradenton for $4,950. Combined with other donations from local people, we are within $1,000 of reaching the matched contribution of $7,000 from the private foundation for the 2017/2018 school year! The happiness of the children is a great short-term success indicator. Long Term Success We are thrilled to have the Backpack Program funded through May The positive feedback from the Principal, teachers, students, parents and community commends the highly successful program. Funding has been easier to secure than with some of our other programs, which indicates the favorable approval of the program. Program Success Monitoring Program Success Examples We constantly look for feedback of the program from the school officials, students and parents. On rare occasions, opened or bad product has been received from Blessings in a Backpack. The occurrences are rare, and the company is quick to issue apologies and contact the family when necessary. We try to examine the food before it is distributed, but since it is pre-packed and 185 are distributed each week, it is not possible to visibly inspect each product. Overall, the program has been very well received and we get numerous compliments and gratitude from staff, students and family. During one of our monthly food pantries, a 10-year-old boy requested to speak with one of the managers of the backpack program. When introduced, he said he asked his mother if he could thank us for everything we do. He said that the backpack program has made him very happy. We are very proud to be able to serve all 185 students on free/reduced lunches at Myakka City Elementary School. Comments Program Comments by Organization 9

10 We are diligently working to try to put together more programs in Myakka city, such as AA meetings, drug programs, ESL, counseling services, and internet access. Our only missing piece is a satisfactory building. We are investigating all possibilities, and have every intention of making this happen. As mentioned earlier, we currently own a portable building, and are searching for appropriate property on which to place it. The additional space will hopefully allow us to add programs, including: AA meetings Migrant family programs, such as ESL Expansion of dental/medical programs Clothing pantry Free counseling services Internet access And many other programs that are currently not available or not conveniently accessible to Myakka City residents. 10

The two groups worked great together and put on a terrific health fair. We look forward to expanding the program and continuing our collaboration with Wee Care. Giving Alliance of Myakka City works with other fundraisers to help people in medical financial trouble. For instance, in February 2012, Myakka City Foundation provided manpower (50 people) to the Levi Dakin Fundraiser - an individual diagnosed with brain cancer. The event netted over $50,000. We collaborated with the Donnie Gibson fundraiser - raffling off tickets for 1/2 side of beef. We raised over $3,000 and the entire event netted almost $40,000! We have participated on many other individual fundraisers. "Christmas Magic in Myakka" - our annual Christmas party - is in collaboration with Myakka City Community Center, and is held at the Myakka City Community Center location. In 2016, we held our first "turkey basket" giveaway. We collaborated with Myakka City United Methodist Church and Myakka City Family Worship Center to identify and provide Thanksgiving baskets to needy in the area. Additionally in 2016, we collaborated with Myakka City Family Worship Center to coordinate a trial "Athletic League" in association with Manatee County Parks and Recreation. This trial collaborative effort will identify whether Myakka City can self-sustain such a league. We are beginning with a soccer league to see interest. Most vital to our information gathering efforts is how many adult volunteers are prepared to dedicate time and talent to the venture. Comments 11

Therefore, it is difficult for him to attend meetings since he spends 75% of his time in Ohio.
