3 SECTION - I PRE-QUALIFICATION FOR SHORTLISTING OF CONTRACTORS FOR CONSTRUCTION OF BRANCH BUILDING OF ICAI AT KANNUR, KERALA IMPORTANT INSTRUCTIONS TO APPLICANT 1. Name of work : Construction of Institutional building for Kannur Branch of SIRC of ICAI at Pallikulam, Kannur, Kerala Estimated cost : Rs.1,78,00,000/- (Rupees One Crore Seventy- Eight lakhs only) (including civil, sanitary, drainage, electrical, landscaping & fire fighting, external developments, roads, interior work, furnishing, generator, transformer etc.) 3. Total construction area 4. Description of the proposed building : sq.ft.+basement : BASEMENT PLUS UPPER _TWO FLOOR, RCC Roof Institutional Building 5. Period of completion : 12 Months 6. Eligibility Criteria : As mentioned at SECTION II 7. Submission of Applications This document contains total Seven Annexures (I-VII), which are to be duly filled in by the applicant. Dully filled in applications in duplicates in sealed envelopes along with colour photographs of important projects completed during the last five years must be sent either by registered post or by courier or delivered by hand at the following address: The Chairman Infrastructure Committee Kannur Branch of SIRC of ICAI Brigade Centre, Fort Road Kannur The last date of submission is on or before 14/12/2016 up to 1500 hrs. The Institute reserves the right to accept or reject any or all the applications without assigning any reasons thereof. Applications received after cutoff date and time will not be considered.

4 8. Qualification Criteria While deciding upon the short-listing of entities, great emphasis will be given on the ability and competence to do quality works within the specified time schedule and in close co-ordination with other agencies. 9. Decision of Institute of Chartered Accountants of India in regard to prequalification of entities will be final. No correspondence in this connection will be entertained. 10. The application shall be signed by person/persons on behalf of the organization having necessary authorization/ Power of Attorney to do so. The Applicant shall sign each page of the application. 11. If the space in the proforma is insufficient for furnishing full details, such information may be supplemented on separate sheets of paper, stating therein the part for the proforma and serial number, separate sheets shall be used for each part. 12. Any letter or document accompanying the pre-qualification form shall be submitted in duplicate. 13. Applications containing false and/or inadequate information are liable for rejection. 14. While filling up the application with regard to the list of important projects completed or on hand, the applicants shall include works relating to commercial/institutional big housing project township etc. 15. Only short listed/ pre-qualified applicants will be informed about the result of the Expression of Interest (EOI) applications and would be invited to the Bid accordingly. 16. Application for pre-qualification should be accompanied by: (i) Type of firm, i.e. Proprietorship, Partnership, Private limited, Limited Company, etc. along with list of its Directors.

5 (ii) List of works completed in the same name and style during the last five years giving the cost, location and dates of commencement and completion for each work with performance certificates and detailed address of the contact persons along with their telephone numbers and IDs (iii) Photographs of two recently executed projects, (iv) Details of works in hand, (v) Copies of (a) Sales Tax Registration Certificate (b) Permanent Account number, (c) VAT/ WCT Registration certificate, (d) Service Tax Registration Certificate (vi) List of equipment and machinery, (vii) Testing facilities (viii) Names and qualifications of professionals and technical personnel with their total experience and period/number of years in the employment with them. (ix) Certificate as a registered Contractor with the, MES, CPWD, any other Central/State Government agency as may be applicable, if any. (x) Documentary evidence of having independent PF Code number from Regional Provident Fund Commissioner. (xi) Permanent Account Number (PAN). (xii) Turnover Certificate (xiii) Copy of IT Return for last 3 years (xiv) Annual Income certified by Auditor (xv) Bidding Capacity The bidding capacity of contractor should be equal to or more than the estimated cost of the work put to tender. The bidding capacity shall be worked out by the following formula: Bidding Capacity = [AxNx2]-B where A = Maximum value of construction works executed in any one year during the last five years taking into account the completed as well as works in progress. N = B = Number of years prescribed for completion of work for which bids has been invited. Value of existing commitments and ongoing works to be completed during the period of completion of work for which bids have been invited. 19. A list of arbitration cases, projects abandoned and legal cases pending in the courts involving the applicant together with the status of the cases may kindly be furnished. 20. If it is found at the later stage that any information furnished by applicant is incorrect or has suppressed any material fact, the applicant shall be liable to be

6 debarred from tendering/taking up the work in full or in part. The Institute may verify the particulars furnished by the applicant independently including the inspection of completed works. 21. Mere submission of application does not entitle for qualifying the short listing process. 22. The short listed /qualified applicants only will be issued tender documents for participating in tender process.

7 SECTION - II ELIGIBILITY CRITERIA 1. This invitation to respond is open to such qualified and reputed contractors/ companies/firms which are registered and have their registered office in India. 2. The eligible entities must be incorporated and registered under the Indian Companies Act, 1956 or The Partnership Act, The parent company of any subsidiary company, which is seeking qualification on the financial strength of its parent company, would have to give a written undertaking that it would bear all financial or contractual liabilities of the subsidiary with regards to this pre-qualification application and the subsidiary has been authorized by them to submit the prequalification application for and on their behalf. 4. The parent company of any subsidiary company, which is seeking qualification on the technical strength of its parent company, would have to give a written undertaking that its technical capabilities/ resources would be available to the subsidiary company as and when required for the completion of the subject contract. 5. Along with the General Eligibility criteria, applicant has to satisfy the following criteria also. (a) Should have satisfactorily completed at least two similar works costing not less than Rs.50,00,000/- during the last five years. Similar work means: Building work with RCC framed Structure, Electrical, Plumbing, external development work, fire alarm and fire fighting system etc. (b) Should have average annual financial turnover of Rs. 40,00,000/- or more on civil construction work during the last three years ending 31 st March, (c) Should not have incurred loss in last two years ending 31 st March, (d) Should have a Solvency of Rs. 80,00,000 certified by their bankers.

ICAI and its authorized representatives are hereby authorized to conduct any enquiries to verify the statements, documents and information submitted in connection with this application and to seek clarification from our bankers and clients regarding any financial and technical aspect. This letter of application will also serve as authorization to any individual or authorized representative of any Institution referred to in the supporting information to provide such information as deemed necessary and as requested by ICAI to verify the statement and information provided in this application such as the resources, experience and competence of the applicant. 4. This application is made with full understanding that: (a) Bids by pre qualified applicants will be subject to the verification of all information submitted for pre-qualification at the time of bidding. (b) ICAI reserves the right to: - amend the scope and value of any contract bid under this project, in such event, bids will only be called from pre-qualified bidders who meet the revised requirements/criteria; and - Reject or accept any application, cancel the pre-qualification process and reject all applications. (c) ICAI shall not be liable for any such action and shall be under no obligation to inform the applicant the grounds therefor. 6. 9 (a) Signed so as to legally bind the Company or all the partners jointly or severally; and (b) Submitted with a consortium/joint venture agreements providing the joint and several liabilities of all partners in the event the contract is awarded to us. 7.
