3 2. COMMUNITY NEED FOR PROGRAM A. Summarize the proposed program and the nature/extent of Rancho Santa Margarita s (RSM) need for the program. Include information regarding the characteristics of persons to be served (e.g., age, disability, income situation, other distinguishing characteristics), and data that supports the unmet need for the proposed program in RSM. Over the past 31 years, Families Forward's Homeless Prevention Program has helped low-income families throughout Orange County maintain self-sufficiency in spite of the prevalance of under-employment and the increase in housing costs. The Homeless Prevention Program provides comprehensive safety-net services to at-risk families including access to our on-site food pantry, counseling, career coaching, and tenant and life-skills education. According to the 21 st Annual Report on the Conditions of Children in Orange County, 49% of OC students (243,432 children) received free and reduced lunch in the 2014/15 school year. In the Saddleback Valley Unified School District, 25.3% of students received free and reduced lunch. This number is widely viewed as a proxy for the number of children at or near the poverty level. In addition, 5.2% of all children in Rancho Santa Margarita are living in poverty with 5.1% of students enrolled in Saddleback Valley Unified School District being homeless. Families are in vital need of support services and we find that many families who have never before had to ask for assistance are suddenly in dire need. Of the 228 Rancho Santa Margarita residents served in the grant year, 69% were extremely low-income, 27% were very low-income, and 4% were low-income, as definited by HUD. 64% of those served self-identified as Hispanic and 9% were disabled. The requested CDBG funding of $6,000 from the city of Rancho Santa Margarita would allow us to continue serving this vulnerable population with vital safety-net services. B. Discuss if other organizations provide a similar service to RSM residents and how the proposed program differs or augments these similar services? Explain why you consider this program to be costs effective when compared to similar services provided by another agency. Families Forward's comprehensive approach to homeless prevention programs is unduplicated in RSM. By combining food assistance with career coaching, tenant education and access to certified counselors, Families Forward is able to assist families in a wrap around manner, enhancing stability and leading to greater propensity of selfsufficiency. In addition, our programs bring hope and joy to children and their parents with new school supplies at the start of each school year and with meals and gifts during the holidays each year. Thanks to the generous amount of in-kind contributions from the community and the dedication of our volunteers, Families Forward's cost effectiveness is quite remarkable with 89% of all finanical contributions directly funding programs and services to client families. In fact, for the sixth year in a row, Charity Navigator has given Families Forward its top 4-star rating, placing us in the top 3% of ranked charities, nationwide. 4 C. Provide the following information regarding the anticipated number of individuals to be served by the proposed program from 7/1/2016-6/30/2017: 1. Of the RSM residents listed above, how many will be assisted with requested CDBG funds? 250 Individuals D. HUD Objectives Create a Suitable Living Environment: Activity designed to benefit the community, families, or individuals by addressing living environment issues. Provide Decent Affordable Housing: Housing activity designed to meet individual family or community housing needs. Create Economic Opportunities: Activity such as economic development or commercial revitalization that creates or expands job opportunities. HUD Outcomes Availability/Accessibility: Services, infrastructure, housing or shelter will be made available/accessible to Low- & Moderate-Income people, including the disabled. Affordability: The activity will provide affordability for Low- & Moderate-Income people including creation/maintenance of affordable housing, basic infrastructure or services. Sustainability (Promoting Livable or Viable Communities): The program/project will improve the community or neighborhoods by making them livable or viable by providing benefits to Lowand Moderate-Income people. E. Regarding the Outcome selected above, describe how success & effectiveness of proposed services will be measured. Include definition of success/effectiveness, tools to measure program success/effectiveness, the % of individuals served that will met the success/effectiveness threshold & how clients will be tracked after they leave the program to measure outcome. If outcome measurements are not in place discuss steps to be taken to implement performance measurements. If awarded the $6,000 CDBG grant from Rancho Santa Margarita, our goal for the program year is to serve 250 RSM residents through a variety of services provided by our Homeless Prevention Program. We expect approximately 150 residents to come to our program center to receive food assistance, allowing these residents to dedicate their limited budget to other essential costs such as rent or utilities. In addition, we will work with liasons in the Saddleback Valley School District to identify and refer at least 100 needy individuals to participate in our seasonal programs including Back-to-School supplies, Thanksgiving food, and holiday gifts from our Adopt-a-Family Program. RSM residents will also have access to career coaching, counseling services and tenant education workshops to increase knowledge and coping strategies to improve their economic and life situations. All services delivered will be tracked in our HMIS-compliant database, ClientTrack, providing a reporting mechanism for tracking the number and type of services provided to RSM clients. 5 3. ORGANIZATION CAPACITY AND EXPERIENCE A. Summarize your organization s experience to carry out the proposed program. For over 31 years, Families Forward has helped low-income and homeless families move beyond overwhelming circumstances to achieve stable, healthy lives for themselves and their children. As one of our core programs, the Homeless Prevention Program has assisted families with food, counseling, career coaching, and tenant education to provide a safety-net to at-risk families. Families Forward is governed by a 30- person Board of Directors made up of executives and leaders from the community who guide the agency in growth and strategic planning, formulate policies and assist in all aspects of fund development. Key program staff includes: Marissa Hall, Volunteer Manager, manager the volunteers who work on-site at Families Forward on a daily basis both in the front office and in the Food Pantry. Isis Cortina, Program Assistant, assists volunteers and clients in the food pantry and front desk with client intake and documentation. She also has a key role in the management of clients during our seasonal programs. Kimberli Claytor, Director of Operations, directs Families Forward's operational functions, including managing the Volunteer Manager, Program Assistant, Facilities Assistants and data entry team. Armene Humber, Certified Career Coach, helps client assess their skills, research job opportunities, create powerful resumes and prepare for job interviews. Rose Rivera, Career Coach, facilitates career workshops in Spanish and works closely with clients on resumes, interview skills, and job searches. B. Families Forward leverages each donations with other vital resources such as volunteer time and in-kind donations to operate a highly effective and efficient agency to deliver safety-net services to vulnerable families. Annually, more than 2,000 volunteers provide over 21,000 hours of service in nearly every aspect of the organization. Homeless Prevention services rely on a significant number of the volunteer hours and donated goods/services. This includes Community Wide Food Drives, food pantry organization and food distribution, donated school supplies and holiday gifts, and the leadership to manage our Back-to-School and Adopt-a-Family programs. 7 4. PROGRAM INFORMATION AND BUDGET A. CDBG funds received by Families Forward will be used to help pay program staff salaries. Families Forward has specifically identified the position of Volunteer Manager whose primary responsibility is to manage more than 2,000 volunteers who contribute over 21,000 hours of services yearly. This position is vital to our ability to efficently and effectively serve RSM residents with critical Homeless Prevention services. C. CERTIFICATION I hereby certify that I am authorized to submit this application for CDBG public service grant funding provided by the City of Rancho Santa Margarita ( City ) by the Board of Directors of Families Forward ( Agency ). If grant funds are granted, funds will be used solely to benefit low- and moderate-income Rancho Santa Margarita residents. Agency understands that general liability, auto liability insurance, and workers compensation insurance are required and will be provided per terms of a grant agreement to be executed between the City and the Agency. Agency understands that grant funds are provided on a reimbursement basis and will provide appropriate documentation to substantiate expenditures submitted for reimbursement. Grant funds will be administered pursuant to this agreement and consistent with applicable federal regulations. If the Agency fails to serve eligible Rancho Santa Margarita residents during the term of the contract, or fails to substantially attain projected accomplishments (defined as at least 75% of projected number of persons to be served), Agency may be required to repay all or a portion of funds already disbursed to the Agency by the City and/or forego receipt of additional grant funds.
