1 You will not be able to save your project in the middle of entering the information. Once you have submitted the form, it is saved and you will not be able to go back and make changes to that project. You can use this Word document to fill in the information you need to at your own pace and come back to the website at a later time and copy and paste the information you entered in your Word document into the online form. Jurisdiction Name: (Select before you begin or you will lose anything you have entered) Anna County Name: You can select multiple counties Collin County Contact Name: Molly McFadden Contact mm@anna.org Agency or Department Address: Biohazard Department Phone Number: Fax Number: Jurisdiction Priority Ranking: (Each jurisdiction must rank all projects in priority order. Projects cannot be assigned the same priority number.) Total Project Funding Requested: (Do not use commas when entering the amount.) 2 $1,000,000 Please check if this jurisdiction is NIMS Compliant. Yes Mutual Aid: Collin County, Denton County, Hunt County, Please list any jurisdictions you have mutual aid with City of McKinney that will be impacted by this project Project Title : Biohazard Sushi Detector Machine Sample Project 1. Provide a summary description of this project and its purpose. Specify what you want to buy and how you plan to use it. (Limited to 200 words; longer responses will be truncated) The City of Anna is requesting $1,000,000 to purchase a Biohazard Sushi Detector Machine. This machine will help Anna and Collin County prepare for and respond to a sushi attack. The machine will sample questionable material used in preparing sushi (fish, rice, seaweed, etc.) to determine if it has been contaminated. Contaminated sushi is the leading cause of upset stomachs among sushi eaters throughout the North Central Texas COG region, including Collin County and Anna. This project will also benefit nonsushi eaters since most sushi eaters are related to someone that does not eat sushi. By protecting the sushi eaters in our region we can better prepare for, and respond to any incident involving contaminated sushi. As a result, the entire North Central Texas Region can benefit from the Biohazard Machine.
2 2. Please check the box if this is a continuous project that has been previously funded. N/A Original Project Title: N/A Grant used to fund this project: N/A 3. List up to four (4) National Priorities this project primarily supports. See pages 4 12 of Grant Guidance Strengthen CBRNE Detection, Response, and Decontamination Capabilities Strengthen Medical Surge and Mass Prophylaxis Capabilities 4. List up to six (6) Target Capabilities this project primarily supports. (Press and hold the CTRL key to select more than one target capability.) See pages of Grant Guidance CBRNE Detection WMD/Hazardous Materials Response and Decontamination Mass Prophylaxis Medical Surge 5. Please choose the primary investment justification for your project. Sustaining and Enhancing CBRNE Prevention and Response Capabilities 6. Describe how this project supports the above National Priorities and Target Capabilities. (Limited to 200 words; longer responses will be truncated) This project falls in line with national and state efforts to protect the sushi eating population from disasters at the local level. By protecting the local sushi eating population, we are directly supporting the state goal of PREVENT and the national Goals of MASS PROPHLAXIS and MEDICAL SURGE enhancement. 7. Please check the box if this is a regional project. Yes 8. Describe the geographic area(s) your project will cover and how it will impact the overall region. (Not to exceed 300 words; longer responses will be truncated) The project will enhance the detection of contaminated sushi throughout Collin County and the NCTCOG region. Although the machine will be located in Anna, It is mobile and will be able to respond to any suspected contaminated sushi incident. The machine can also be used during regular scheduled inspections of sushi restaurants to detect any potential problems prior to being consumed. 9. Describe what the potential Homeland Security risks of not funding this project are and discuss how the implementation of this project will decrease or prevent those risks. (Not to exceed 500 words; longer responses will be truncated) The risks of not funding this project can result in a burden on our regions medical system due to the large population of sushi eaters. Many potential sushi emergencies go undetected each year and are therefore often unreported. By funding the regional Sushi Biohazard Detection machine, the region is furthering it s commitment to clean food, happy sushi eaters, and over better family and community relationships.
