2 Instructions The Full Proposal is the second step in the RFP application process. It should be formatted as follows: A Microsoft Word Document Typed in clear, legible font (e.g pt. Arial, pt Calibri, or pt Verdana) Single spaced One-inch left /right margins Not to exceed 12 pages Make sure you refer to the current Healthy Communities RFP before you begin preparing your proposal document. 2

3 Proposal Project Summary/Narrative Abstract Please copy and paste your project summary/narrative abstract from the online portion of your application. The following categories should be maintained as headers as you proceed to write the proposal. Problem or Need Up to 10 points Describe the specific barriers to equitable access to healthy eating, active living and tobacco prevention that your proposed project seeks to address. Please include data, information, or other evidence that documents this need. Be specific about how the problem/need impacts the proposed target group(s) and their environments (e.g. home, neighborhoods, schools, etc.). It is highly recommended that you use current local or regional data. One potential resource can be found at kchealthmatters.org. Brief History of Organization Up to 5 points Include current programs and services. Explain the fit between organization s mission and the proposed project. Keep in mind that those who will be reviewing your proposal are from outside the Kansas City area and do not know your organization nor its reputation; therefore, the historical overview of your organization is your opportunity to convey your successes and to make the case for why your organization would be the right one to implement this project. Equitable Engagement & Access Describe how your project utilizes the following examples, if applicable: Empower community members to be active change-makers in building healthier communities, environments and policies; Build on best practices and promising approaches that increase the well-being of communities, improve organizational practices or enact policy change; Increase access to and affordability of healthy and safe environments; Support efforts to help ensure that environments are tobacco-free. Mobilization for Action Describe how your project utilizes the following examples, if applicable: Strengthen core organizational operations and sustainable systems; Increase awareness and skills of individuals and communities through culturally relevant education; Organize and equip individuals and organizations to take action or influence policy; Build public will that catalyzes and accelerates the field. Multi-Sector Collaboration Describe how your project utilizes the following examples, if applicable: Reach out to non-traditional partners to create alignment and a stronger unified voice; Align strategically with local, regional or state coalitions and networks; Connect people and communities across multiple fields and sectors. 3

4 Staffing and Organizational Capacity Up to 10 points Describe the background and expertise related to the community culture of the individual(s) who will perform the work. Indicate whether the positions for which you are requesting funding are already filled or will be new hires. If the position is not filled, please describe the qualifications and experience you are seeking in the individual. Identify the relationship of the lead contact for this proposal to the applicant organization. Further describe the organization s capacity, in addition to staffing, to complete the project and achieve results, e.g. experience with the participant population, experience with similar approaches or projects, etc. NOTE - If your organization has cultural competency policies, you may attach them as a separate document which will not be counted in the Narrative Section page count. Sustainability Up to 10 points What is the plan for continuing the work and adding value to community health after the HCF grant ends? Describe how various sources of support (financial, programmatic, in-kind, personnel, facilities, etc.) complement one another in the proposed project. Please describe how sustainability at the organization and community level will be achieved, including specific sources of support. Project Timeline Up to 5 points Provide a timeline for the proposed time period. If you are requesting multi-year funding, include the rationale explaining why success of proposed project requires funding for more than one year. Outcomes & Evaluation Over the past six months, HCF has been working with a strategic consulting firm, Informing Change, to develop a theory of change (TOC) for our Healthy Communities round. This process has enabled us to be more explicit about both our strategies for this grantmaking round and our expected outcomes. In this Theory of Change, HCF has identified both short and long term outcomes we hope to accomplish through this funding round. While there are many outcomes listed and we don t expect grantees to be working towards all of them, we do expect that the outcomes listed in the proposal will align with the TOC. Once outcome(s) have been identified, make sure and detail how they will be measured. For example, how will you determine any changes in access to healthy and safe environments? 4

5 Explain what data will be used and who will be responsible for managing and collecting that data. Describe how the project utilizes an equitable and inclusive approach to evaluation that engages the voices of affected community members. Just as importantly, elaborate on how the information will be used to monitor and/or improve the project in an ongoing way; also describe any examples of using data to determine what wasn t working and any adjustments made. When describing the project and its intended outcomes, the proposal is strengthened by the inclusion of visual representation such as a logic model. 5

6 IMPORTANT - The following items are not counted in the Proposal Document page count. Submit each attachment separately; do not combine attachments into one PDF. Supporting Attachments 1. Combined Budget Worksheet & Justification, a combined format template provided on the HCF website. When describing other sources of funding for your project, please indicate whether the funding source is secured (S) or pending (P). When explaining the expenses for your request, please indicate the amount you are asking HCF to fund per each line item. For salary coverage, please enter the % FTE for each position HCF is being asked to fund. It is recommended that you further explain or clarify your budget line items in a narrative format in the text boxes that are provided. If you are requesting two-years of funding, please fill out both the Year 1 and Year 2 tabs on the Excel spreadsheet*. *Use the HCF Application & Reporting Budget Template found on our website 3. Non-profit applicant organizations or fiscal sponsors/agents, please include copies of your organization s: Certificate of incorporation IRS Non-Profit Determination Letter Copy of organization s most recent IRS 990 Report (nonprofit tax return) Most recent audit Roster of your board of directors and information about the board s demographic composition related to race and gender Board-approved operating budget NOTE All documents submitted to HCF are available to the public; therefore, please omit board members addresses, s, and phone numbers 4. For governmental entities that are the applicant organization or fiscal agent, please include copies of: Enabling statute/legislation or official description of the entity s responsibility or purpose Operating Budget Most recent audit, and List of elected and/or appointed officials who oversee the entity s performance 2. Letters of Commitment. If the proposal is a joint application, each organization that will receive a portion of the grant funds must provide a Letter of Commitment on the organization s official letterhead. The Letter must state the organization s commitment to the project and indicate the organization s specific role in the project and its share of the grant proceeds. Letters of Commitment are required to confirm the dollar value of In-Kind gifts of space, staff, supplies and equipment. Letters of Support from partnering organizations that will not share in grant proceeds - include as attachments. 6
