1 City of San Luis Obispo Human Relations Commission Grants-in-Aid Program PURPOSE The City of San Luis Obispo Human Relations Commission (HRC) Grants-in-Aid Program provides financial support to non-profit organizations that promote the economic and social well-being of its citizens. REQUIREMENTS Organizations that received Grants-in-Aid during the funding cycle must have complied with all reporting requirements to be considered for the grants. Grants are primarily made to organizations based in the City of San Luis Obispo. Agencies based in neighboring communities that serve a significant number of City residents also are encouraged to apply. Grants are made only to non-profit organizations. Organizations must comply with the HRC Bylaws which creates an environment within the City in which all persons can enjoy equal rights and opportunities regardless of race, religion, sex, national origin, sexual orientation, age, physical, mental or economic status. HRC Bylaws can be found on the City s website here: Applications for a specific program, project, equipment need or general operational support will be considered if the organization s mission falls under one or more of the HRC priority funding areas listed below. Enter into a contract with the City of San Luis Obispo prior to receipt of GIA funds. A sample can be found on the website at PRIORITY FUNDING AREAS The following priority funding areas are not listed in order of importance: Homeless prevention including supportive services and transitional housing Hunger and malnutrition prevention Supportive physical and mental health services Services for seniors and/or people with disabilities Supportive services for youth and their development; especially targeted to youth at risk Special consideration will be given to projects for which matching funds have been secured. In addition, programs are encouraged that will be used to initiate a new, ongoing service; that train or support service providers in order to expand their scope of services and/or increase the numbers of residents served; that initiates a new, ongoing funding source for the organization; that direct services efficiently to needy residents with low overhead and administrative costs.
2 CHECKLIST: Submitting your application Applications must satisfy all of the eligibility criteria in order to be considered. Please verify by checking boxes below that this application satisfies each eligibility criteria: I have used a standard typeface of 11 points or larger in every document submitted. I did not attach materials other than those requested. I did not use staples, binders, or covers (Paperclips or binder clips suggested). I have included the following documents, both in hard copy and in a complete, signed scanned and ed packet in.pdf format: Grant Application List of the organization s board members. Organizational chart. Copy of document certifying Federal tax-exempt status. Copy of project budget. Copy of organization s most recent complete fiscal year financial statement (for the previous year because it is the middle of the current fiscal year) including a statement of financial position/balance sheet, statement of revenue & expenses/income statement, profit & loss. I understand that my grant submission should include every document listed above, following the format listed above, submitted in two ways: - One (1) single-sided hard copy submitted to 990 Palm St. San Luis Obispo, CA 93401, either in-person or by mail. If submitted by mail, it must arrive by 5:00 p.m. February 4, Postmarks will not be accepted. - One (1) signed application submitted via to kauslen@slocity.org in PDF format. In other words: print every document listed above, sign the signature page, then scan all of it, together, as one complete packet. Treat this as if you were not submitting a hard copy and you need to submit all the same information by in order for your application to be considered. Frequently Asked Questions: Q. How can I submit my grant application if I do not have a scanner? A. You can go to office stores such as Kinko s, Staples or OfficeMax to have your application scanned and saved on a CD or flash drive or ed to you. Q. How can I submit my grant application if the file is too big to ? A. There are a couple options. 1) You can save it as a zipped file by right clicking on the document wherever it is saved on your computer and go to send to>compressed (zipped) folder. 2) If you have Adobe Acrobat Pro, you can go to File>Save as>reduced size.pdf Sidenote: offers this software at a discounted rate of under $50. 3) You can also save the file to a CD and send it in with your hard copy application.

Agency representatives attend to provide input on funding priorities. October 2014 October 7 th City Council approves priorities based on HRC recommendation from the 2014 Community Needs Workshop November 2014 November 5 th HRC Meeting (finalize GIA application, etc.) December 2014 Early December sent to past GIA grant applicants, United Way, and the Community Foundation advising them of the upcoming grant process December 8 th GIA applications available online January 2015 Early January Human Resources staff will send an to all former GIA applicants with a reminder of the Feb. 6, 2013 deadline and to schedule a technical support meeting for application submission. February 2015 February 4 th GIA applications due September 2014 September 3 rd HRC conducts Community Needs Workshop in Council Chambers Late September CDBG Applications available October 2014 October 7 th City Council approves priorities based on HRC recommendation from the 2014 Community Needs Workshop Mid-October: CDBG Application Deadline December 2014 December 3 rd HRC Reviews and votes on CDBG Funding recommendations. January 2015 County Publishes Draft Action Plan 30 day review (CDBG) February 2015 February3 rd Council hearing/adoption of CDBG Funding Recommendations
4 GIA February 4 th HRC assigns subcommittee to review GIA applications CDBG March 2015 GIA Subcommittee meets to review applications and develop preliminary recommendations March 2015 County Board of Supervisors Approves 2015 Action Plan April 2015 April 1 st - HRC reviews GIA Subcommittee recommendations and adopts preliminary GIA recommendations Mid April sent to GIA applicants noting preliminary recommendations and inviting them to attend the public hearing at the May 6 th HRC meeting. May 2015 May 6 th HRC public hearing.
