Programs that reduce and treat overweight and obesity. Also programs that focus on prevention through healthy eating and physical activity. Chronic Conditions. Submit Complicated Proposals Forget to submit required attachments Create unattainable objectives Use acronyms Include your accounting department as FA Forget to click submit on the application YOU WILL RECEIVE A CONFIRMATION OF SUBMISSION IF YOU DON T, THAT MEANS YOU HAVE NOT PROPERLY SUBMITTED CALL ME IF THIS HAPPENS IMMEDIATELY! 17

23 Sponsorship Program Application Requirements Completed (online) application & attestation sheet A copy of your organization s tax exempt status documentation A list of your organization s board members or governing body and their affiliations (ie Kristin Weber, Kaiser Permanente) If using a fiscal agent, a letter from fiscal agent on their letterhead and signed by their executive director or president, explaining the relationship between the two organizations. Application Process Submit the above documents via IGAM links provided 23 January 5, Kaiser Foundation Health Plan, Inc. For internal use only.
24 Sponsorship Program Review Process All requests (application & attachements) must be received at least 60 days prior to the event Once received, will be sent from IGAM confirming receipt of request Requests are screened and forwarded to review committee for consideration Please allow up to 90 days for notification on the status of your request 24 January 5, Kaiser Foundation Health Plan, Inc. 25 Sponsorship Program What Happens if your Organization Receives Funding It may take up to six weeks for funding to be issued after approval of request As a recipient of a donation through the Sponsorship program you are required to submit the following: Completed Acknowledgment of Cash Contribution form within 5 days of check receipt by organization Report within 30 days of event on your letterhead, to include: Organization name and event title Number of Attendees Report on how proceeds were used List of sponsors 25 January 5, Kaiser Foundation Health Plan, Inc.
