This addendum shall hereafter be regarded as part of the solicitation. Items not referenced herein remain unchanged, including the response date. NOTE: FAILURE TO USE AN ADDENDUMS REVISED BID/PROPOSAL FORM SHALL DEEM BIDDER NON-RESPONSIVE. Page 22 of the solicitation indicates a Submission Deadline of Thursday, 1. October 4, Please confirm that the deadline is Tuesday, October 16, 2018 as indicated on the cover page. The correct deadline is October 16, If sub contractors are listed on the submittal, will they need to complete any of the required forms? No, only the firms submitting are required to fill out the forms. Can you advise on the number of references the County would like submitted with the proposal? Please see page 24, Form #3, 5. Three (3) Reference responses are to be returned with the proposal package. Will the County specify the lab to be used or will the lab be a sub selected by the successful firm? The lab would be selected by the firm and act as a sub. 5. Is the Cover Page included in the 10 page limit? No, it is not included in the 10 page limit. Page 1 of 3

2 6. Are we allowed to include a front and back cover for our response? Yes you are. 7. Can we include a Table of Contents in our response? If yes, will that count towards the 10 page limit? It is not necessary if you are using Tabs/Section dividers as outlined on page 21, Does our proposal need to be numbered sequentially? The submittal requirements that are listed starting on page 21, should be the format used for how your proposal should be submitted Page 7 of the RFP, Section 16.1 states The prime Proposer on a solicitation may not also be listed as a sub-proposer/consultant/contractor to another firm submitting a proposal for the same solicitation. Sub-Proposers/consultant/contractor may be listed on multiple proposals for the same solicitation. However, Section 17.1 states The use of sub-proposer/consultant under this solicitation is not allowed without prior written authorization from the County representative. Can the County please confirm if we are allowed to include subconsultants in our proposal submission? You are allowed to use subconsultants in your proposal submission. Page 10 of the RFP, Section states the default contract term shall be one (1) year with three (3), one (1) year renewals for a total of four (4) years upon mutual written agreement of both parties. However, on page 15, Section 2.1 states the contract will be for a two-year (2) period, with the option for extension or renewal for three (3), additional one (1) year periods. Can the County please confirm the contract period for this contract? Page 10, states Unless otherwise stated in the scope of work, specifications, or special conditions. What is state on page 15 of the Special Conditions is the Project Term. 11. Who are the incumbents for this contract? Please click on this link to see who the current incumbents are: The RFP states that the consultant is not pursuing one or more of the Tasks (1-12) written justification will be provided. However, the scoring criteria and weight table indicates that the Criteria 2 (Experience with coastal studies and projects) has a weight of 35 points. So, if we don t submit for Task 2.9, how would we be deducted 35 points? Or, does this Criteria just represent your firm s experience in the Tasks they are submitting on? Please see the attached adjusted criteria. 13. Are subs allowed? Yes, subs are allowed. Page 2 of 3

3 14. Verification of coverage is it needed with submitting the proposal? No, it is not needed with your submission. 15. Reference Survey do you want the references survey filled out from our references while submitting, or does Lee county send it out to the references to fill out? Please see page 24, Form #3, 5. Three (3) Reference responses are to be returned with the proposal package. 16. Are we able to use fonts small than size 10 font for graphics, tables, call out boxes, etc? Anything smaller than a size 10 font is unreadable and not suggested to use. BIDDER/PROPOSER IS ADVISED, YOU ARE REQUIRED TO ACKNOWLEDGE RECEIPT OF THIS ADDENDUM WHEN SUBMITTING A BID/PROPOSAL. 4 Ver 04/12/ Addendum 1 SUBMITTAL REQUIREMENTS & EVALUATION CRITERIA 1. SUBMITTAL REQUIREMENTS & EVALUATION CRITERIA 1.1 Interested firms shall include the following information in their submittal responses to this solicitation. The following format and sequence should be followed in order to provide consistency in the firm s responses and to ensure each proposal receives full consideration. Use 8 ½ x 11 sheet pages only with minimum font size of 10 points and with tabs or section dividers to separate sections as defined below. More than one section is permitted on one page unless otherwise indicated below. Undesignated information shall be inserted at the rear of each package. Place page numbers at the bottom of every page, excluding dividers. Proposal documents should not contain links to other web pages; such links will not be reviewed for evaluation purposes. 1.2 Submittal package may not exceed 10 pages printed single-sided; page restriction excludes required forms found herein and dividers. PLEASE INCLUDE PAGE TABS/SECTION DIVIDERS so that those evaluating your submittal can easily compare each section with others that are submitted. If any of the information provided by the Proposer is found to be, in the sole opinion of the Evaluation Committee and Procurement Management Director, substantially unreliable their proposal may be rejected. 1.3 Proposers shall submit one (1) original hard copy (clearly marked as such) and one (1) electronic version on a USB flash drive set containing the proposal submittal in an unlocked PDF format. The County may request specific files be submitted in specialty format (IE: Provide a Project Timeline in Excel format.) Vendor shall accommodate such specialty requests as stated within the submittal requirements describe herein. Should files not be provided in the format or quantity as requested Vendor may be deemed Non-Responsive and therefore ineligible for award. In case of any discrepancies, the original will be considered by the County in evaluating the Proposal, and the electronic version is provided for the County s administrative convenience only. Limit the color and number of images to avoid unmanageable file sizes. Briefly discuss relevance of those similar projects to Tasks listed in this solicitation. If you are not responding with experience on all Tasks, please state so and the reason why. Scoring will be based on the experience the firm has in their area of expertise.
