5 2019 Behavioral Health Initiative The Behavioral Health Initiative program: Supports bold ideas to improve access to mental health and substance use disorder services. Supports strategies and service models that will improve prevention, early identification and intervention, and treatment. Addresses at least one of our two cross-cutting goals of integration and workforce. Maintains focus on improving measurable health outcomes for children and/or older adults. 5

11 Concept Papers No more than two single-spaced pages (Word or PDF). Please include the following sections in this order: Project title and organization name Brief overview of the initiative, including proposed impact Which of the cross cutting goals do you plan to address Key collaborative partners, if any Information about potential sustainability Estimated budget 11 Integrated care for Kids by XYX organization This project will develop a new comprehensive health care home, with a strong focus on behavioral health services, for youth you who are victims of human trafficking. The development of this new site aligns with our organization s experience in providing comprehensive care for youth and adults in the county. The number of youth trafficking victims in our community has increased significantly in the past two years and the county s health and human service body has identified the development of a holistic and trauma informed health home for those impacted to be a major priority. Primary collaborators on this project include ABC and DEF organizations. Based on anticipated billable revenue for the majority of those served, we believe the project will nearly breakeven within 2 years of operation. The project will cost approximately $325,000. This will include $150,000 for therapists, $100,00 for primary care capacity, $50,000 for consultation on policies and procedures, $15,000 for evaluation and $10,000 for marketing.

15 Evaluation Strategy for Your Project 1: Outputs and outcomes matter! Outputs: 15 community residents showed up to our training session. The project served 15 children. Outcome: 85% of residents in attendance indicated that they learned something new that they will apply to their lives. 2: Tell us how you ll know you were successful. Evaluation is simply about how you know what you re doing is working. 15
