1 1 IMPERIAL COUNTY 4-H AWARD APPLICATION The 4-H Award Application County Level Judging. should be filled out by every member whose book is submitted to 1. Fill in and mark the award you are applying for. The appropriate box should be checked for the Achievement Award, the Agriculture Award, Leisure Art Award, Family and Consumer Science Award, or the Novice Award. You must also check the County Medal Program, if you want a project judged in the record book competition. 2. The Novice Award is only for 1st year members. 3. You may only apply for ONE Imperial County 4-H Award per year. (in addition to County Medal Program) 4. May be achieved once as a Junior Member and once as a Senior Member for all 4 awards. 5. There are four Junior winners and four Senior winners for the Achievement Award. 6. There is one Junior and one Senior winner for Agriculture, Leisure Art and Family and Consumer Science Award. 7.
