1 School Grants for Healthy Kids School Breakfast for Healthy Kids-Sustainability Grant Instructions and Checklist Application for Funds due Friday, April 5, 2019 Action for Healthy Kids is pleased to release its School Breakfast for Healthy Kids grant for the school year. These grants are for schools and districts to expand efforts started by previously funded AFHK breakfast granted schools for alternative breakfast initiatives. Schools will be awarded $1,000 based on project, ability to offer breakfast at no charge and impact of grant. Funded schools will also receive expertise and people power to help implement a successful project that leads to sustainable change. Confirm eligibility with your State Coordinator or Regional Manager Steps to Apply 1. Register and participate in the School Breakfast Grant webinar. In this webinar, you'll learn about specific grant details and eligibility requirements and also receive helpful tips for applying. This webinar will be archived for those unable to attend the live session. Apply for a School Breakfast Grant Thursday, February 21, :00 PM (ET), 3:00 PM (CT), 2:00 PM (MT), 1:00 PM (PT) Register Here 2. Action for Healthy Kids recommends that schools complete the paper application, using the grant application instructions and checklist, with their School Health Team in order to organize the proposal in a thoughtful manner, check spelling and ensure accurate character count. On average, the application takes less than 60 minutes to complete. 3. Proceed to the online School Portal to submit your official application by copying and pasting your responses. Applications must be submitted online via AFHK s School Portal. Paper applications will not be accepted. 2 School Breakfast for Healthy Kids Grants School Breakfast for Healthy Kids grants provide funding and technical assistance for schools and districts to implement new alternative breakfast programming. Funding will be awarded in the following manner: $1,000/school: To be considered for $1,000, schools will: o Offer breakfast at no charge for ALL students (either participation in Provision 2 or Community Eligibility Provision) o Have been awarded a school breakfast grant by AFHK in the school year o Have a current breakfast participation between 55% and 75% Awarded schools will: Implement an alternative breakfast model: o A school breakfast best practice is to move breakfast out of the cafeteria and after the bell. The following models are examples of alternative breakfast models: Alternative Breakfast Model Definitions Breakfast in the Classroom: Breakfast delivered to classrooms; students consume breakfast in classrooms after the bell. Grab and Go to the Classroom: Breakfast is picked up by students at high traffic location(s) at the school (not the cafeteria) and taken to the classroom to eat. Grab and Go to Other Locations: Breakfast is picked up by students at high traffic location(s) at the school (not the cafeteria) and taken to other locations (not cafeteria and not classroom). Second Chance Breakfast: Breakfast is offered at a later time or after first period/homeroom. Students may pick it up at alternate locations (not cafeteria) or from cafeteria (if all students have access and time to eat). Other: Schools can create other models. The best practice is to move breakfast closer to students. Schools funded for $1,000 will offer breakfast at no charge for all students. o No charge breakfast reduces stigma related to eating breakfast. Provide information to students and parents on the importance of school breakfast Complete AFHK s School Health Index at the beginning and end of the grant cycle Join the celebration during Every Kid Healthy Week by hosting a family-friendly healthpromoting event during April 2020 Priorities Funding priorities include: Applications that demonstrate permission from administrators to allow students to eat breakfast in their classroom. Applications that demonstrate principal support for proposed changes.

3 Application Checklist The following checklist provides clarification on application questions. Using this list, check off each point to ensure you submit a complete application. Action for Healthy Kids recommends that schools complete the paper application first, but ALL applications must be submitted online via the School Portal to be considered for funding. Section 1: Contact Information Question Contacts for the school/district and grant. 1 Applicant Contact Information The primary grant contact for all grant requirements including the midterm and final report as well as the School Health Index. Role - Select one from the given list. Phone Number Primary We will use for all communications. Please ensure there are no typos in your address. Alternative We will use this address if we are unable to reach you with your primary address. At times, we communicate after schools are out for the summer. If you are not a year-round employee, please provide another (or provide an for a staff member that will be active in the summer). Section 2: School Project Data Details Question School Breakfast Data - Information collected on school level 2 School Meal participation (October 2018 Data) Gather key data points below to create a baseline for participation growth. Most districts report this information to state agencies and should be able to provide this information easily. If completing for multiple schools, setting up a spreadsheet with the data points for each school may make your entry in the portal easier. Total # of lunches served Enter numeric value Total # of breakfasts served Enter numeric value Days of service If days of service is different for breakfast and lunch, enter the days of service for breakfast. Enter numeric value. Enrollment Enter numeric value Average Attendance Average number of students attending school during month. Cannot exceed enrollment. Some districts report as a percentage (ex. 96.1%);

4 please multiply the percentage by enrollment to get the average number of students attending. Enter numeric value. Free and Reduced % for school (if CEP, enter 100), (i.e. 85% = 85) Enter numeric value. 3 Please indicate your current breakfast model (select one): Choose option that best describes your current breakfast model. If your school implements a combination of more than one model, please select which model serves the majority of children. If none of the options describe your current breakfast model, please describe your model (100 character limit). 4 Please indicate which alternative breakfast model will be used to drive increased participation in school breakfast. (select one) The primary goal of this grant is to help serve more breakfasts to more students. Evidence has shown that schools that provide breakfast in the classroom are more likely to increase breakfast the most. If your school implements a combination of more than one model, please select which model serves the majority of children. If none of the options describe your current breakfast model, please describe your model (100 character limit). If your project is changed from previous year s funding, please check which model here and then provide more details in your project description. 5 Will your school continue to offer all students breakfast at no charge in the school year? Options: Yes, through participation in CEP/Yes/No Providing breakfast at no charge is a best practice for schools with high free and reduced percentage (often 60% or greater). No charge breakfast reduces stigma. Only schools offering breakfast at no charge will be considered for the $1,000 award. IF NO, please don t continue this application, as this grant is only for those continuing free breakfast for all students. 6 Do students have permission to eat breakfast in their classroom or homeroom/first period class after the bell? Eating in areas other than the cafeteria, which can often be a barrier to participation, is a key strategy for success. AFHK is more likely to fund a project where students have permission and support for eating in the classroom. Establishing that prior to applying is important. If you need additional resources connect with your state coordinator. 7 Identify the grade levels that receive nutrition education. (Check all that apply) This question applies to the school and not the entire district. Consult your health teacher, garden coordinator, school nutrition staff, classroom teachers, parent group representatives, etc. to ensure you provide accurate information.

5 Section 3: Project Summary Question Project Summary 8 Describe in detail the breakfast project you wish to incorporate at your school to increase breakfast participation. Action for Healthy Kids is looking for utilization of best practices that lead to the greatest impact on policy, systems, and environmental changes and thus, on student health and academic achievement. Limit 2000 characters. This is the chance to describe what you want to do and why you believe it will increase participation. Include potential timeline to demonstrate the project throughout the school year. 9 Once the grant term is over, describe how you will ensure that grant efforts and success will be sustained. (1000 characters) We would like to know that project and equipment will be in place beyond the grant year, ideally for three years. Section 4: Budget Question Budget 10 Start Up/Implementation needs: (check all that apply): Schools will receive either $2,000 or $3,000. Awards will be determined by need, current breakfast participation and project plans (check all that apply). Funding is NOT to be used for staffing or for food (except for food for a taste test) Awarded schools will receive 65% of funds with the 1 st installment check in August The remaining 35% of funds will be awarded in January 2020 (after the submission of the Midterm Report and pre-school Health Index). Items provided in the drop down are all valid uses of funding. We recommend schools put no more than 10% towards incentives. AFHK does not collect receipts for expenditures. 11 Describe other funding your school has received (or applied for) to address school breakfast activity. We know that schools require funds to purchase the necessary equipment to start new breakfast programming. Please identify if you have received any other funding to support your program and from what organization(s). If you are using district funding to supplement grant funding please let us know that, too.
