1 KICK-OFF PACKET Friday, May 15, 2015 Important Walk-A-Thon/Field Day Dates: Wednesday, April 22 nd Begin collecting sponsorships We are excited for the annual Walk-A-Thon/Field Day Event which will be held this year Friday, May 15 th. Enclosed is the Kick-Off packet with all the necessary forms for each student to participate (ie sponsorship forms, prize sheet, student lunch order form, volunteer form). All students are encouraged to obtain sponsorships for this event. They will be sponsored for their participation, not based on lap count. Our goal is to have 100% participation any amount collected will benefit our school directly. Be sure to check out the great prizes students are eligible to win based on sponsorship levels. NEW this year... online giving! You will now be able to make your sponsorships online by visiting: choosing School Walk-A-Thon from the drop down menu. Friday, May 8 th Final day to turn in lunch order forms to school Thursday, May 14 th Final day to turn in sponsorship money to school be eligible for prizes Friday, May 15 th Walk-A-Thon/Field Day Event!!! Students should wear grade level t-shirts along with comfortable shorts & shoes. Don t forget your sunscreen, beach towel for picnic lunch, hats/visors etc. As always, we will need many parent volunteers to make this day a success. To volunteer, the Sign-up Genius link can be found on the IHM School PTO website. For babysitting volunteer boxed lunch options, please see the volunteer info sheet included in this packet. 2 PRIZES The day will conclude with an assembly where students will have a chance to win prizes based on their sponsorship levels. Over 35 individual prizes will be given away! Raffle Prizes Regular Raffle: $5-$25 gift cards Options: Orange Leaf, Starbucks, Smoothie King, itunes, Amazon, Target Mega Raffle: Choose One - ipad Mini, Beats Studio Headphones or a $300 Kenwood Towne Center Gift Card INDIVIDUAL Sponsorship Level Prize FAMILY Sponsorship Level Prize $15 1 ticket for regular raffle drawing $25 1 day pass for out of uniform 2 tickets for regular raffle drawing $50 3 day pass for out of uniform 4 tickets for regular raffle drawing $60 1 day pass out of uniform per students 2 tickets for regular raffle drawing per student $100 1 week out of uniform pass per student 2 tickets for regular raffle drawing per student 1 ticket for Mega Raffle per student $75 1 week out of uniform pass 6 tickets for regular raffle drawing 1 ticket for Mega Raffle 8th Grade Students: Any 8th grade student that has an individual sponsorship $50 or more OR a family sponsorship of $100 or more will receive an out of uniform pass for the REMAINDER of the YEAR starting Monday, May18. Homeroom Contest: Grades K-8: Homerooms with 90% participation will win an extra recess with a treat.

If you choose to order, please fill out form below return with payment to school by Friday, May 8 th c/o Walk-A-Thon Lunch. Checks should be made payable to: Immaculate Heart of Mary School, online payment is not an option for lunch. NOTE: The cafeteria will be CLOSED for the Walk-A-Thon/Field Day Event on May 15 th so, students that choose to not order a lunch will need to bring a bagged lunch & drink on this day. 6 VOLUNTEER LUNCH/CHILDCARE Please sign up to volunteer thru the Sign Up Genius link found on the school PTO website. We will need volunteers to facilitate each of the Field Day stations. Rain date will be May 22. *Reminder: All volunteers will need to be current with their Virtus certification. Walk-A-Thon/Field Day Schedule: Grades K-6: 8:30-11:45 Grades 7-8: 1:00 2:30 All School Lunch: 12:00 1:00 If you would like to join the students for a picnic lunch before or after your shift, you are welcome to pack a bagged lunch/drink or order a box lunch.
