Interested parties may download the Proforma, terms and conditions and other details from our website under Tender Section up to ,.2013, 01:00 PM. Proposal should reach to us on or before , 2013, 01:00 PM. Please note that submission of proposals does not automatically qualify for empanelment. 3 GENERAL RULES AND INSTRUCTIONS TO THE INTENDING APPLICANTS: 1.0 Interested contractors may collect the Document for empanelment by downloading the application from the tender section of the bank s website The duly filled in application downloaded from the website, should be submitted at the following address after enclosing the application fee amounting to `1000/- (Rupees One Thousand only) by way of Demand Draft / Banker s Cheque favouring Bank of Baroda, payable at Bareilly. The details of the applicants and their experience shall be furnished in the prescribed Application Format only. Wherever required and if the space provided is not sufficient, particulars can be furnished in Annexure but such details shall be clearly mentioned in the respective columns of the Application Format. Applicant shall enclose latest copies, brochures and technical documentation giving additional information about the applicant. 4 4.0 No costs incurred by the applicant in applying, in providing necessary clarifications or attending discussions, conferences or site visits will be reimbursed by the Bank. 5.0 Documentary proof with respect to the details furnished in the application form regarding eligibility criteria shall be furnished along with the application form. In this regard, r copies of work order and completion certificate and/or such documents shall be submitted. Incomplete applications or applications without proper proofs for establishing their credentials will be summarily rejected and no correspondence will be entertained tained in this regard. 6.0 If, information and details furnished by applicants are found to be false at any time in future or any information withheld, which comes to the notice of the bank at a later date, the empanelment of such applicant will be cancelled immediately. 7.0 Applications received after the due date and time is liable for rejection. 8.0 Bank of Baroda reserves its right to reject any / or all the applications without assigning any reasons whatsoever. 9.0 Those who have applied previously are also requested to re-submit the application along with the latest details for further processing The firms/contractors declared black listed in past by Bank of Baroda need not apply Empanelment does not guarantee placement of order/entrusting the work. Bank laid down norms/procedures will be followed for placing order/entrusting work. Eligibility Criteria for f OOH Media 1. Agencies must have in same business for at least 5 years and should have specialty in the field of outdoor publicity preferably for Banking Sector. 2. Turnover should be `0.50 crore or more in the same activity. 3. Should have full-fledged office with Multi Media Facilities at Bareilly/other major centers of Zone. 4. Should have own site/authorized sites for display of hoardings/glow signs etc. at different important places at Western UP & Uttarakhand Zone. I/We have read the instructions appended to the Performa and I/We understand that if any false information is detected at a later date, any future contract made between us and Bank of Baroda, on the basis of information submitted by me can be treated as cancelled by the Bank. 2. We agree that the decision of Bank of Baroda in selection of enterers will be final and binding on me/us. 3. All the information furnished in the attached Performa is correct to the best of my/our knowledge. 4. I/We also agree that I/We have no objection if enquiries are made about the works listed by me/us in the accompanying sheets or any other enquiry on the information furnished herewith in the accompanying sheets. 5. We understand that any addendum/corrigendum will be issued on the Bank s Website only and the bidder has to refer the same before final submission of the tender.
