1 Purpose To acknowledge and honor exemplary efforts, above and beyond assigned job responsibilities, of a member of the university community who promotes, fosters and demonstrates understanding and appreciation for advancing and celebrating diversity and inclusion at UNF. Deadline Nominations and applications will be accepted until Monday, April 4, 2016 at midnight. Mailed applications must also be postmarked by Monday, April 4, 2016 at midnight. Awards There will be one award for $2,500, for an individual. Call for Nominations/Applications The UNF Commission on Diversity and Inclusion is extending "An Invitation to Submit Nominations and Applications." The application form is available online at Eligibility All UNF faculty, staff and students are eligible. Nomination Criteria Nominations may be made by any person and self-nominations are acceptable as long as eligibility criteria are met. Nominators should describe the nominee's commitment, as demonstrated by their exemplary efforts. Nominee Material Completion of the nominee application, which includes a personal statement and two letters of reference, is required by the stated deadline. Screening and Selection Procedures For purposes of the award selection process, diversity and inclusion initiatives are defined as organizational programs, activities and efforts, designed to promote and support differences in people and society that may include race and ethnic origin, gender, disability, religion, age, education, sexual orientation or other differences. The selection process includes: review of nominations received, by the deadline, to verify that minimum eligibility criteria have been met and forms have been properly completed; interview of finalists conducted by the Selection Committee and determined by a combination of ratings; review of the Selection Committee's recommendations by the Diversity and Inclusion Initiatives Committee and the Executive Committee of COD I; and selection of a winner by voting members of the UNF Commission on Diversity and Inclusion. Award Notification The selected nominee will receive notification of the award May 6, 2016 and receive subsequent notification of the monetary award no later than June 26, However, public acknowledgement and the recipient s award presentation will be made during the 2016 fall convocation, by the university President or designee.

Please address the following: any current diversity effort(s) or program(s) that promote diversity and inclusiveness; and a summary of your defined goals, actions steps and accomplishments toward achieving a work or learning environment that recognizes, promotes and encourages a welcoming and positive organization for everyone (qualitative and quantitative narrative). Two (2) letters of reference are required to complete your application. Personal Statement Describe why diversity is important to you. Limit your statement to 250 words or less.
