1 Powell County Schools 691 Breckenridge Street Stanton, Kentucky Invitation for Bid Closing Time to Receive Bids: Tuesday, March 22, 2016 at 11:00 am EST Any bids received after this date and/or time will not be considered and will be returned unopened. Powell County Schools is soliciting sealed bids for a contract covering the following services. The District reserves the right to renew this contract for up to four (4) additional (1) year periods. Installation of data drop ports for wireless access points (APs)in ceilings and associated cabling Installation of data drops for classrooms/offices for computer workstations Installation of access points By signing and submitting a bid and upon acceptance of an awarded contract, the vendor agrees to the following provisions: All work is contingent upon E-Rate and local funding. Payment will be by USAC Discount Method. All work is contingent upon the goals of the District and/or the District Technology Plan. The service provider shall have a Service Provider Identification Number (SPIN) with the Universal Service Administrative Company (USAC) for the Federal Communications Commission. The vendor shall be properly authorized under the laws of the Commonwealth of Kentucky to conduct business in this state and shall remain in good standing to do business in Kentucky for the duration of any contract awarded. Background checks shall be provided by the Vendor for all installers upon award of the bid. If applicable, all components/services shall comply with: o USAC Eligible Services requirements o Minimum requirements of the Kentucky Educational Technology System (KETS) o Kentucky Occupational & Safety Health Act (K.O.S.H.A.) Standards o Family Educational Rights and Privacy Act (FERPA) o KRS 45A the Kentucky Model Procurement Code All project schedules for each site shall be approved by the school system. Prices are to remain fixed for the period of the contract and the 4 additional 1 year renewals. After a contract is awarded, all submissions from vendors shall be a matter of public record. The Powell County School District reserves the right to reject any and all bids if they do not serve the best interests of the board.

2 Sealed bids must be submitted by U.S. Postal mail, FedEx, UPS, or in person by Tuesday, March 22, 2016 at 11:00 am EST to: Powell County Board of Education c/o Julie Clark 691 Breckenridge St Stanton, KY The bid package must be clearly labeled on the outside: Bid Package Form A (attached) must be included with the sealed bid. Bids must be submitted in ink or typewritten. The Solicitation, the bid specifications, and any Addenda shall be posted to the Powell County Schools website at Building maps for erate projects shall be provided through the EPC website and shall not be posted to the district website. Specific building maps for additional projects, over the life of the contract, shall be provided via or in person to the vendor. Sealed bids shall be opened at 11:15 am EST on Tuesday, March 22, 2016 at Powell County Central Office, 691 Breckenridge St., Stanton, KY The winning bid shall be submitted for board approval at the next board meeting. The initial term of the contract is anticipated to be July 1, 2016 through June 30, The District reserves the right to renew this contract for up to four (4) additional (1) year periods. The winning bid shall be chosen by the lowest evaluation bid price. The criteria is as followed: Service Ethernet Network Cabling Criteria for Evaluation Total Points Possible Cost of Goods/Services 45 Prior Experience 10 Ineligible Cost Factors 10 Reciprocal Preference for bidders 35 Total Points Possible 100 Any questions must be submitted to the District contacts named in this solicitation. Questions may be submitted in writing via or orally by phone. The District will respond to relevant questions in writing by issuing an Addendum to the Solicitation. 3 DISTRICT CONTACTS: Kelly Noss or John Estep 40 Bruen Avenue, Stanton, KY An optional vendor conference/walk through will be held on Tuesday, March 1, 2016 at 9:00 am EST beginning at the Powell County Middle School. The contract, and renewals if applicable, shall include installation in any of the following buildings of the Powell County School District: Bowen Elementary Clay City Elementary Stanton Elementary Powell County Middle School Powell County High School Powell County Academy Powell County Central Office Powell County Annex Powell County Bus Garage Powell County Maintenance Building The initial term of the contract (July 1, 2016 June 30, 2017) will include cabling at the following: Bowen Elementary 23 drops Clay City Elementary 22 drops Stanton Elementary 20 drops Powell County Middle School 141 drops Powell County High School 13 drops Vendor s bid shall be broken down into costs for each site and a total cost. All cabling will include the following Scope of Work: Wiring plans/maps with exact locations and number of drops shall be provided specific to each project over the life of the contract. Installation Twisted Pair CAT 6, or higher cabling, shall be routed to patch panels from each AP and from computer workstations located in different areas of the building. No run will be over 320 ft. Cabling shall be hung above ceiling using hangers and not laid on the ceiling tiles. Cabling should follow existing runs whenever possible. Cabling shall be connected to patch panels and labeled according to naming scheme provided by Powell County Schools Data drops to computer workstations shall be terminated, either in block walls or onto drywall with raceway, to 4 slot port boxes. Data drops to APs will be terminated in ceiling above each AP with a 2 port RJ45 surface mount box. All cabling shall have a four foot service loop at the endpoint and a four foot service loop at the closet.

4 All installed cabling and associated components shall have a minimum warranty of 1 year after completion of install. Vendor shall test cabling for gigabit per second Ethernet. Vendor shall provide an accurate electronic map of cabling routes and final naming scheme. Any software, license(s), setup, training and support needed must be included in the bid. Vendor shall provide all components required for routing, mounting and termination of cabling. Powell County Schools shall provide needed cables to connect port boxes to AP and/or workstation and to connect patch panels to switches. All access point installations will include the following Scope of Work: Powell County Schools shall provide the access points and hanging brackets. Vendor shall install/mount access points securely and in locations specified by the district.
