1 Gwinnett Braves Grant Application Form The Atlanta Braves Foundation is the non-profit arm of both the Atlanta Braves and the Gwinnett Braves that actively supports community programs. Operating under the Foundation, the Gwinnett Braves specifically focus on awarding monthly grants to deserving 501c3 organizations in Gwinnett and its surrounding counties. The grant program offers the only monetary donations approved by the Gwinnett Braves. Each month, the grant s selection committee will focus on a specific area of giving according to the designated theme outlined. Applicants that best fit the theme and fulfill the mission of the Foundation will be awarded a $1,000 grant, 50 Gwinnett Braves tickets and opportunities to raise awareness for their cause in partnership with the Gwinnett Braves. In an effort to recognize all applicants and to raise both awareness and funds for their respective causes, the 2014 season will introduce a new Non-Profit Partnership Series. The series will follow the monthly themes of the grant program and give special acknowledgement to grant recipients from August 2013 through July Participating organizations will be offered Gwinnett Braves tickets and opportunities to both fundraise and advertise during these events. Dates for the Non-Profit Partnership Series will be set prior to the 2014 season. Below is a list of the monthly themes. 1

2 A completed checklist is required in order to be considered for a grant. Please check all themes you feel your organization s grant request matches so that your application will be reviewed during each appropriate month. Applicants do not need to re-apply until one year has passed from the date of submission but are encouraged to send additional information that shows why your organization would be a great choice for the current month s theme. Themes are purposefully broad so that organizations will likely qualify for multiple months. The theme can fit your organization s mission, a particular program or annual fundraising event. January I Have a Dream Organizations that grant wishes, dreams and opportunities for the youth, elderly, injured or sick. February Heart Month Organizations that bring awareness and/or raise funds for heart related diseases and illnesses or that promotes a healthy diet and fitness programs for children. March Salute to the Arts Organizations that support art programs or education. April Take Me Out to the Ballgame Organizations that promote youth athletics or activities. May Cancer Awareness Organizations that bring awareness to cancer prevention/detection or that raise funds for cancer research. June Summer Safety Organizations that fund or run safety programs for children. July Red, White and Blue Organizations that support the military or military families. August Back to School Organizations that support education or after school/mentoring programs. September All About Kids Organizations that benefit children. October Four Legged Friends Organizations that benefit animals. November Thanksgiving Organizations that work to end hunger and/or homelessness, that support children and families that are homeless or that run and maintain a food bank. December Season of Giving Organizations that benefit youth, education or sports programming. 2

References (Corporate and or Financial) REQUIRED MATERIALS TO SUPPORT APPLICATION Please include copies of the following documents with your completed application. Applications submitted without these materials will be considered incomplete and not reviewed for grant funding. * Copy of the Internal Revenue Service letter, ruling tax-exempt status under Section 501 (c)(3). * List of the board of directors of the organization, including primary business or professional affiliation of each (addresses and phone numbers are not required). * Donor list of individual, corporate, and foundation contributions and the amounts granted during the past twelve months. * Complete budget for the program, including itemized unearned income sources, both committed and pending. The organization s operating expenses should be highlighted. * Financial statement from the most recent fiscal year (audited preferred). * Annual report, if available. Applications are accepted on an on-going basis. All materials must be received by the Gwinnett Braves by the 15th of the first month for which you wish to apply.
