1 Northwood Neighbourhood Services The 7 th Installment of the Share the Light Awards CALL FOR NOMINATIONS Celebrate the best of the best! Paying tribute to those hardworking individuals in the non-profit sector who, through their leadership, tireless delivery of programs and services, and their commitment to enrich the community, have helped improve the lives of others. Celebrate with the Award Recipients in the categories of: Leadership Service Excellence Client Services Volunteer Excellence And introducing The Community Impact Award

2 ABOUT THE AWARDS Community nonprofit agencies play a vital role in supporting and strengthening individuals, families and communities. Northwood Neighbourhood Services developed the Share the Light Awards program to recognize the passion and hard work that often goes unnoticed. It is important to pay tribute to our Peers for their dedication and commitment to those who need help and support. The awards recognize the enormously important roles played by leaders, staff and volunteers of our nonprofit organizations in improving our lives and communities, as well as exemplary clients who have made best use of the resources that they have received. Five awards: have been created, and will be presented at the 7th Installment of The Share the Light Awards Gala to be held on Friday September 21, Leadership Award: Created to recognize Executive Directors and Board Directors who work tirelessly to lead their organization with vision and passion Service Excellence Award: Created to recognize Frontline Workers who go above and beyond in delivering services to their clients Volunteer Excellence Award: Created to recognize those dedicated volunteers who give generously of their time to support their agency s mission and vision. Client Services Award: Created to recognize Clients who, as a result of agency programs, was able to achieve, maintain & enhance a state of physical, mental & social wellbeing. Community Impact Award: Introduced and created to recognize an individual or an entire organization reaching above and beyond the call of duty to positively impact the community. 3 APPLICATION INFORMATION: There is a separate nomination form for each award. Please complete the form for the award you wish to nominate. We encourage you to include relevant background and supporting documents such as brochures, press releases, newsletters, etc. Please provide a description of any specific project(s)/activities being submitted for recognition RULES AND REGULATIONS: The decision of the judges is final. All submitted entries are the property of Northwood Neighbourhood Services and may be used by Northwood for whatever purpose in connection with the Share the Light Awards or its promotion. Northwood Neighbourhood Services retains the right to publish the name of nominees and recipients. All entries must include full contact information for nominee. Photos must be submitted as part of the entry cannot be returned and remain the property of Northwood. All nominees must agree to be photographed and must be available to receive the award at Northwood s Gala event. Eligible organizations: Not for profit community service agencies in the Greater Toronto Region are eligible to apply for the Share the Light Awards. Nominations are open to all permanent or contract agency staff that have been with the agency since January Achievements must cover the years beginning January 2016 to December Clients must have accessed your agency s programs or services within the last 18 months. 8 SERVICE EXCELLENCE AWARD Nomination Form Page 1 The Share the Light Service Excellence Award was created to recognize Frontline Workers (Such as Settlement Workers/Counselors and Community Workers) who go above and beyond in delivering services to their clients. CRITERIA: To be eligible for the award the nominee must: Have ongoing relationship with one or more non-profit organization within the community. Regularly display compassion and humility toward all community members. Create a passion project or program that directly and positively impacts the community they serve. Be aware of the issues facing the community while creating and implementing solutions or strategies to affect change. Display leadership like qualities which impact the community around them.
