Attendees include 3RNet members and representatives from other state-based agencies involved in workforce issues, the federal Office of Rural Health Policy, Health Resources and Services Administration, National Health Service Corps, Veteran s Administration, and Indian Health Service. 3RNet is committed to ensuring this event helps you make meaningful connections. 4 Hotel Information Sleeping rooms at The Inn at Opryland (2401 Music Valley Drive, Nashville, TN 37214) will be available at the conference rate of $139/single and double, plus applicable taxes (currently 15.25% + $2.50). Reservations: Mention Group Name: 3RNet Book your group rate for 3RNet Annual Conference here. Exhibitor Drawing A drawing for prizes will be held on Wednesday, September 14. Drawings allow maximum opportunities for attendees to stop by each table with a check-off sheet (requiring your initials) to be eligible for prizes. Each Exhibitor will be invited to draw the winner s name for their prize. Attendees must be present to win. Each Exhibitor is required to bring a small gift (suggested $100 value). Please indicate your item to be donated on the registration form. Suggested items include gift cards, small electronic devices, etc. Please do not bring gift baskets because of the difficulty in transporting them. Again this year, we will include drawings for Exhibtors to win a gift from 3RNet. Drawing will be held immediately following drawings for attendee prizes. Exhibitor/Sponsor must be present to win. Acceptance of Application and Assignment of Space Applications for exhibit space are subject to review by the Exhibitor/Sponsor Committee. Space assignments are made on a first-come, first-served basis until all tables have been assigned. No assignments will be made until payment is received via check or credit card. Applications must be received by May 1, Those received after May 1, or after exhibit space is filled, will be placed on a waiting list and will be notified if space becomes available. Payment and Cancellations Applications for exhibit space must be accompanied with the exhibit fee in order to confirm table assignment. Please make check payable to 3RNet. No application will be processed without this fee. No refunds will be made for space not utilized during the conference.

5 Request for Participant Addresses Sponsors will receive a copy of the participants names and addresses at the conference. Sponsors will also receieve an excel spreadsheet post-conference with updated information on attendees who registered onsite. Shipping and Handling Address all shipments to: The Inn at Opryland; Attn: 3RNet Annual Conference, Vendor Name; 2401 Music Valley Drive; Nashville, TN Any shipment that is received greater than one day prior to the event date will incur storage costs: Letter/Pak Size: $5.00+/day, 0-15 lbs.: $5.00+/day, lbs.: $12.00+/day, lbs.: $15.00+/day, lbs.: $19.00+/day, lbs.: $25.00+/day, pallet: $ /day. Necessary Language Exhibitor assumes the entire responsibility and liability for losses, damages and claims arising out of injury to persons or damage to exhibitor s displays, equipment or other property brought upon the Premises of The Inn at Opryland Hotel and agrees to indemnify, defend and hold harmless 3RNet, The Inn at Opryland and its owners, servants, agents and employees against all claims or expenses for such losses including reasonable attorney s fees arising out of the use of the hotel premises excluding any liability caused by the negligence of 3RNet or The Inn at Opryland or its owners, servants, agents or employees.
