1 United Way National Capital Area 2014 Community Impact Grant Award Letter of Agreement United Way of The National Capital Area ( United Way NCA or UWNCA ) Community Impact (CI) Committee is pleased to be able to provide funding to your organization ( Grantee ) and continue our partnership to work together to achieve our shared community goals. The Board of Directors of United Way NCA has approved the funding award of Amount: $0.00 to Agency: Your Agency's Name for Program: Your Program Name in Region: Your Region This award is for one year beginning January 1, 2014 and terminating December 31, This grant is payable as outlined below. As a condition of receiving the grant, Grantee agrees to adhere to the following terms and conditions. Funds will be distributed in three (3) payments starting in February 2014 in the following structure: Distribution 1 = 40% of total grant award sent February 15, 2014 Distribution 2 = 30% of total grant award sent July 31, 2014 Distribution 3 = 15% of total grant award sent October 31, 2014 Distribution 4 = 15% of total grant award sent January 31, 2015 Grant award distributions are contingent upon grant reporting submission and contract compliance. The payments will be separate from your agency's designation payments. Grantees Receiving Award Amounts Different Than Requested Grantees receiving award amounts different than requested are required to resubmit the sections of the 2014 United Way NCA Community Impact Grant Application that will be affected by the change in funding. The adjustment to the grant application must be received, reviewed and accepted before officially signing this Grant Agreement. To sign, or update and sign, your 2014 United Way NCA Community Impact Grant Application, please sign back in to your online grant application on the same website you submitted your Letter of Intent (LOI) and original grant application ( Use the and password you created when filing your LOI/2014 grant application. If you have forgotten your password, you can also reset it at that time. 1

2 2014 United Way NCA Community Impact Grant Award Reporting Criteria The grantee must submit two (2) reports to the United Way of NCA via the Online Grant Reporting Tool. We will provide your agency with the report template one month prior to the dates below. Each individual grant award requires a separate grant report for each region. Interim Grant report due July 25, 2014 for January 2014 July Final Grant Report due January 23, 2015 for the full grant period of January 2014 January These reports must be completed and submitted in full by the due dates to avoid delays in subsequent fund distributions. Failure to submit interim and final grant reports on time may result in your ineligibility to be considered for future Community Impact Grants. Interim and final grant reports will be used during subsequent Community Impact Fund Grant decisions as a basis for future grant awards United Way NCA Community Impact Grants reporting criteria for both the interim and final grant reports is used to track implementation and progress of programs supported by grant dollars, ensure maximum utilization of grant dollars, and the achievement of measurable results as compared to the expected results reported in the 2014 Community Impact Grant Application. Access to the Online Grant Reporting Tool will be sent to the agency s grant representative. Interim and final reports must include the following information to be considered complete. Additional information on reporting criteria will be provided prior to report due date. Please report any change in the agency contact information or grant manager, directly through the membership portal or send your changes to membership@uwnca.org. All reported information will be compared to program activities and goals submitted in original 2014 CI Grant Application. Program Expenses describe how the CI Grant funding is being utilized to achieve expected program results, including specific expenditures and how those items are tied to the program success. Target Population include who and how many individuals are being served with this funding as compared to originally reported number, with explanation of any differences. Demographics of those served - Indicate number of individuals served during interim and final report periods: o unduplicated individuals/families o total number of clients served whose income is below 200%, 125%, and 80% of the Federal Poverty Income Guidelines o number of children under 5 years of age, children under 10 years, children under 18 years o adults years of age, Seniors 55+ years of age, Veteran status, gender, race, etc. o include other demographics of those being served as collected by each program. Program Activities, Inputs, Outputs, and Outcomes describe all program activities/services, inputs, outputs, and outcomes that contribute to program success, including: o number of volunteers, volunteer hours, and volunteer activities o how many were served by each activity, who was served by each activity, how activities served these clients, and o how did this activity change the lives of those served 2

3 Success Story - Provide one success story for each region of programming that was funded with community impact grant dollars. A separate success story is to be reported for each region. TERMS AND CONDITIONS OF GRANT 1. Funds are to be used exclusively for the program and services outlined in the 2014 Community Impact Grant Application. Funds may not be used, or applied to or redistributed for any purpose, service or program other than in the approved application. United Way NCA should be notified in writing no later than 30 days post change if, for any reason, you as a funded agency are not able to provide the program services stated in the application. United Way NCA reserves the right to request reimbursement of funds not used in accordance with the approved program budget. 2. The Funding period is for one year beginning January 1, 2014 and terminating December 31, Funds will be distributed in three (4) payments starting in February 2014 in the following structure: Distribution 1 = 40% of total grant award sent February 15, 2014 Distribution 2 = 30% of total grant award sent July 31, 2014 Distribution 3 = 15% of total grant award sent October 31, 2014 Distribution 4 = 15% of total grant award sent January 31, 2015 Grant award distributions are contingent upon grant reporting submission and contract compliance. Failure to submit interim and final grant reports on time may result in your ineligibility to be considered for future Community Impact Grants. The payments will be separate from your agency's designation payments. 3. United Way NCA may conduct ongoing monitoring of 2014 funded programs. At least one site visit may be conducted during the funding period, to be scheduled in advance with the funded agency. Site visits may include United Way NCA staff, Board members and Regional Councils. Funded partners should be prepared to demonstrate adherence to this agreement and that programs and services are being implemented and tracked as outlined in the approved application. United Way NCA reserves the right to evaluate the data collection and reporting process used by the funded agency. 4. The funded agency will conduct an on-site workplace campaign through United Way NCA to assist in United Way NCA fundraising. United Way NCA staff will provide training, materials, and support. 5. The funded agency will maintain its listing in the database ( and update it annually at a minimum. If the agency does not yet have a listing on the database one must be established by February 28, The funded agency will identify itself as a United Way NCA Member Agency with adherence to the United Way NCA Membership Co-Branding requirements. Further, grant recipients understand that by being deemed eligible as a campaign participant, all grant recipient(s) are required to identify its affiliation with United Way NCA in every practical manner; this includes the proper use of the United Way NCA logo. We are supplying you with instructions and tools to acknowledge the funding you receive, which will help show the link between United Way NCA support and the excellent work you do. Any media or communication that recognizes other funding sources must include also recognize United Way NCA. Please visit for information regarding proper use of the credit line and logo. 3

4 Grantee agrees to indemnify, release, and hold harmless UWNCA and any of UWNCA s current and former directors, officers, employees, agents, representatives, volunteers, service providers from and against any and all costs, losses, claims, damages, expenses, and liabilities, including reasonable attorney s fees and costs, they incur, whether in relation to a third party or in relation to Grantee, Grantee s successors or assigns, or any of Grantee s current or former directors, officers, employees, or agents, that arise out of this grant or any consulting, advisory, or management assistance services UWNCA provides to Grantee, either directly or indirectly through a service provider that volunteers with UWNCA or that UWNCA pays to provide services to Grantee. This clause is expressly intended, without limitation, to effect an indemnification, waiver and release, to the fullest extent permitted by law, of any claim of negligence liability against the aboveindemnified parties as a result of their providing consulting, advisory, or management assistance services to Grantee. No part of this clause extends to the service providers that UWNCA pays to provide service to Grantee. 7. As a matter of policy, UWNCA is making the grant with the understanding that Grantee does not discriminate against people seeking either services or employment based on race, sex, religion, age, sexual orientation, or physical disability. Grantee shall inform UWNCA if this understanding is incorrect before signing. 8. UWNCA may terminate the grant agreement at any time if: Grantee does not provide any of the information or documents as required under the terms of this contract; Grantee files for bankruptcy or any member of the Grantee s leadership team is convicted of a felony; Grantee misstates, conceals or fails to disclose any material information in this contract, or in any written statement or oral examination or hearing, in connection with this contract; or Grantee fails to advise UWNCA within five (5) Business Days if there is any change in the information or representations provided by the Grantee in its proposal after such proposal was submitted to the UWNCA or in the contract agreement after the date Grantee signs this agreement; a determination that the Grantee is not responsible is made in accordance with Law(s) or UWNCA regulations. MANDATORY GRANT MEETING Each grant recipient is required to attend at least one Mandatory Grant Meeting. At least one representative from each organization is required to attend. During the meeting, we will review grant recipient requirements listed within your Grant Award Notification Agreement and will address the following: Community Impact Grant Agreement Contract - Changes/feedback to our 2014 grants process - Grant payment - Data and outcomes training - 6-month and End-of Year reporting - UWNCA branding expectations 4

5 ACCEPTANCE OF THE 2014 UNITED WAY NCA COMMUNITY IMPACT GRANT AWARD Each agency will be required to complete the online 2014 United Way NCA Community Impact Grant Agreement and enter their online signature to accept the award. One agreement is required for each separate 2014 United Way NCA Community Impact Grant Award (e.g. one per region). By signing, your organization agrees to use the award solely for the purposes approved in the grant application, confirms its understanding of the terms of this grant agreement, and agrees to comply with the aforementioned conditions. Failure to complete the online signature acceptance will be viewed as a rejection of the grant award.
