Memorial Dr. Tulsa, OK REGISTRATION: Bring into Office or Mail $30 Fee pre-registration or $40 day of registration If sending check, please make it payable to the Tulsa 66ers. TIME: Registration Check - in 9:00 a.m. - 9:30 a.m. Warm-up/Learn Routine 9:30 a.m. - 12:00 p.m. Lunch Break 12:00 p.m. 1:00 p.m. Try-Outs 1:00 p.m. 1:50 p.m. First Cut 1:50 p.m. 2:00 p.m. Final Round (interviews) 2:00 p.m. 2:30 p.m. Final Round (audition) 2:30 p.m. 2:50 p.m. Final Cut 3:00 p.m. (Check-In will occur at a table where auditions will be held) ATTIRE: Half top (any style, any color) Shorts (any style, any color, any length) NO long pants or sweat pants allowed Shoes - jazz shoes, dancer sneakers, or athletic brands No excessive jewelry, cover all visible tattoos Hair worn down and makeup - come to impress, game day REQUIREMENTS: Female High School Graduate Must be 18 years or older If selected, must have means of transportation You must be able to attend all rehearsals & games JUDGING: Our judges will focus on the following: 1. Positive attitude, showmanship, personality, and energy. 2. Physical and personal appearance. 3. Dance technique and style. REHEARSALS: All rehearsals for the Tulsa 66ers Dance Team will be Wednesday evenings from 7pm to 10pm, possible Sunday evening rehearsals 6 to 8 pm. THE DANCE TEAM SCHEDULE IS VERY BUSY. A TOTAL COMMITMENT IS REQUIRED!

4 DANCE TEAM "WAIVER AND RELEASE" The undersigned requests that she be allowed to demonstrate her skill and ability as a contestant during the auditions for the Tulsa 66ers In consideration of being afforded such opportunity, the undersigned hereby assumes the risk of injury inherent in such activity and waives any claim for damages against the Tulsa 66ers or its personnel for any injuries that may be sustained by the undersigned during such demonstration. In recognition of the media coverage of the Tulsa 66ers and applicants thereto, the undersigned grants the Tulsa 66ers and its licensees her permission and authority to use her name, voice, picture, and likeness in connection with any and all publications, media broadcasts, promotional photographs, promotional posters, commercial products, including but not limited to calendars, team pictures, T-shirts, and any other commercial items. In further consideration of the Tulsa 66ers allowing the undersigned to audition for the Tulsa 66ers Dance Team, and in the event the undersigned is offered employment to become a member of the Tulsa 66ers Dance Team, she hereby understands and agrees that as a condition of employment, she must sign the standard Tulsa 66ers Agreement and provide written authorization under the Fair Credit Reporting Act for the Tulsa 66ers to conduct a background check. The undersigned understands and agrees that the Tulsa 66ers and/or its authorized agents may conduct one or more background checks to obtain information about her current and past employment, education, activities, character, reputation, personal characteristics (including age), and mode of living, including, but not limited to, academic, residential, achievement, performance, conduct, disciplinary, attendance, criminal, credit and driving records, from various federal, state, and local governmental, law enforcement, and motor vehicle agencies, consumer reporting agencies, previous employers, schools, companies, corporations, partnerships, and other entities or persons. This information will be used to evaluate the undersigned as an applicant for employment and, if hired, for any other employment purpose. The undersigned further authorizes any person, entity or agency contacted by the Tulsa 66ers or its authorized agents to furnish the above-mentioned information. If a background check reveals that the undersigned has provided inaccurate information or omitted information on the Tulsa 66ers application, then it may result in the rejection of her application or, if already employed, in the termination of her employment. 5 DANCE TEAM AUDITIONS AND 2006/2007 SEASON PERSONAL INJURY WAIVER AND RELEASE In consideration of the registration of the individual named herein and the opportunity for that individual to voluntarily participate in the 2006/ 2007 Lady 6ERS Dance Team Audition and season if selected, I,, hereby release and discharge the TULSA 66ERS, Aspen Athletic Clubs, Expo Square Pavilion, the Tulsa 66ers Dance Team Choreographer, other sponsors, and promoters from all liability from any and all claims for personal injury and property damage, which may result because of my participation in this event. The undersigned does also hereby authorize medical treatment of said undersigned in the event of injury and does authorize any attending physician to render any and all medical care, which said physician deems necessary and the undersigned accepts financial responsibility for said treatment.
