The host Faithful Navigator is Gary Gornik. All Assemblies in the Northern Illinois District are invited to participate in this Exemplification. Overnight Hotel Rooms are $ plus tax per night. Mention that you are with the Knights of Columbus for these rates. Cut-off date for Hotel Room Reservations is Oct 1, 2011 Phone number for the Rock Falls Hotel is Candidates initiation fee $ includes Banquet Ticket, Social Baldric, and 4 th Degree Pin. Priests ONLY initiation fee $50.00 includes Banquet Ticket, Social Baldric, and 4 th Degree Pin. Sir Knights, Ladies and Guest Banquet Ticket is $ Ladies Luncheon is $20.00 NO REFUNDS ON BANQUET TICKET IF IT IS NOT USED.

2 Page 2 Deadline for Candidates Form 4 s is October 10, 2011 for names to be listed in Souvenir Book. Deadline for Banquet Reservations is October 10, Special Meal requests (if necessary for Medical Reasons or Vegetarians) for Candidates, Sir Knights, Ladies and Guests should be included with Banquet Reservation Form. Special Reminder: A Candidate must be a 3 rd Degree Member in good standing and have a full Six Months in the Order (based on 1 st Degree Date) to be eligible for the 4 th Degree Exemplification. Please be sure to send only COMPLETE packets (Form 4, Dinner Reservation Form, and Check made payable to the Master at least 2 weeks prior to the Exemplification Date. INCOMPLETE PACKETS or PACKETS NOT HAVING ALL INFORMATION WILL BE RETURNED TO THE FAITHFUL NAVIGATOR. Faithful Navigators and Faithful Comptrollers should insure all required information on the Form 4 is completed including: Membership Number First Degree Date COUNCIL FINANCIAL SECRETARY SIGNATURE Name of Assembly, Assembly Number and Location Candidate is joining Faithful Navigator Signature Faithful Comptroller Signature Faithful Navigators are also asked to insure that all Assembly Members and Candidates are made aware that PROPER ATTIRE MUST BE WORN to the Exemplification. Proper Attire consists of: BLACK TUXEDO PANTS AND JACKET, WHITE TUXEDO SHIRT WITH LAY DOWN COLLAR (NO WINGED COLLARS OR COLLARLESS SHIRTS), BLACK BOW TIE, BLACK SOCKS AND BLACK SHOES.

Please note that the dress code for the banquet is black tie or suit and tie for those who are not Sir Knights.
