61 HONORS & GRADES At the end of each term, the GPA is computed for each student based on the grades earned in all full-credit classes completed. Points are given to the grades received: A = four points, B = three points, C = two points, D = one point, F = zero points. The point total is divided by the number of full-credit subjects taken. The quotient is the GPA. Class members are then ranked on the basis of their grade point average. Partial credit courses will not be included in the grade point. A partial credit course is anything less than.5 units of credit per term. This GPA and rank is always included on official transcripts. Report Cards are issued at the end of each nine-week term; mid-term reports will be distributed to students during the 5 th week of the term. Parents can access student grades at any time using the Parent Portal. CHS HONORS, STUDENT AWARDS PROGRAM 2th grade students who excel scholastically are recognized at the annual Student Awards Program in the spring. Remember that a student spectator represents his/her school the same, as does the athlete. 2. Recognize that the good name of the school is more valuable than any game won by unfair tactics. 3. Respond with enthusiasm to the calls of the cheerleaders for yells in support of the team, especially when the team is losing. 4. Accept the decision of officials without question. 5. Recognize and applaud an exhibition of fine play or good sportsmanship on the part of all participants. 6. These recommendations are based on the following: ) Participation requirements; 2) Regular attendance; 3) Meeting the standards established by the high school athletic associations and Central High School. ATHLETIC PHYSICAL EXAMINATIONS The State Department of Public Instruction requires all inter-scholastic athletes to be physically examined ANNUALLY by a licensed physician. Student athletes CANNOT PRACTICE OR PARTICIPATE until the examination is completed and a physician-signed Physical Examination/Insurance Card signed by the parent is on record in the Activities Office. We strongly urge all athletes to have examinations by their own family physicians. A Physical Examination/Insurance/Parental Permit Card is available in the Activities Office. BLUE DEVIL ATHLETIC PARTICIPATION CARDS Davenport Central Athletic Office now requires each student athlete to complete the NEW Blue Devil Athletic Participation Card before each sports season. Each student will need to check in with the athletic office before the start of each sports season to verify that there sports physical and parent acknowledgement forms are current and up to date. If both forms are current, the student will then and only then receive a stamped Blue Devil Card to give to their coach. Without a fully stamped Blue Devil Card the student will not be able to participate in that sports practices or games. This card has been designed to improve our accuracy of our student athletes data and minimize errors. ATHLETIC INJURIES/INSURANCE The following is the statement of the athletic director concerning injuries: A team physician is paid a fee for the emergency care of athletes while participating in secondary interscholastic athletics. The department assumes no financial obligation beyond the emergency care provided by the team physician. Emergency care is defined as care which is limited to first aid treatment only. Any further treatment will be the financial responsibility of the individual player s parents or guardian. All athletes participating in interscholastic athletics, except football, are encouraged to buy the school insurance policy. Insurance policies covering football injuries will be made available to football players when they first report for practice. ATHLETIC ACTIVITIES Central High School offers 2 interscholastic sports. () Each contestant shall be passing all coursework for which credit is given and shall be making adequate progress toward graduation requirements at the end of each grading period. Grading period, graduation requirements, and any interim periods of ineligibility are determined by local policy. For purposes of this sub rule, grading period shall mean the period of time at the end of which a student in grades 9 through 2 receives a final grade and course credit is awarded for passing grades. (2) If at the end of any grading period a contestant is given a failing grade in any course for which credit is awarded, the contestant is ineligible to dress for and compete in the next occurring interscholastic athletic contests and competitions in which the contestant is a contestant for 30 consecutive calendar days.* d. However, a student who engages in athletics during the summer following eighth grade is also eligible to compete during the summer following twelfth grade. Extenuating circumstances, such as health, may be the basis for an appeal to the executive board which may extend the eligibility of a student when the executive board finds that the interests of the student and interscholastic athletics will be benefited. f. All member schools shall provide appropriate interventions and necessary academic supports for students who fail or who are at risk to fail, and shall report to the department regarding those interventions on the comprehensive school improvement plan. g. A student may not participate with or against high school graduates if the graduates represent a collegiate institution or if the event is sanctioned or sponsored by a collegiate institution. Nothing in this sub rule shall preclude a student from participating in a one-time tryout with or against members of a college team with permission from the member school s administration and the respective collegiate institution s athletic administration. [This is NOT a substantive change; it clarifies the college squad rule.] i. No student shall be eligible to participate in any given interscholastic athletic sport if the student has engaged in that sport professionally. j. The local superintendent of schools, with the approval of the local board of education, may give permission to a dropout student to participate in athletics upon return to school if the student is otherwise eligible under these rules. k. Remediation of a failing grade by way of summer school or other means shall not affect the student s ineligibility. All failing grades shall be reported to any school to which the student transfers. *Study tables are required for non-passing grades (see website for policy). CHS ATHLETIC EXPECTATIONS: For student athletes who have a D or F in any class, they will be required to attend a study table. If students do not attend the study table they will have additional consequences. RESEARCH PAPER FORMAT GUIDELINES Secondary Language Arts Department Header, Heading, and Title A research paper does not need a title page; however, an instructor may require one. If so, follow that instructor s directions. To set up your paper, open a Word document. Begin by clicking on View. Select header/footer, but use only the header. In the header window, type your last name, capitalized correctly; hit the space once; insert the page number, using the insert page number function; and right justify. (Use buttons to move header to the far right.) Close the header function. For the heading, beginning one inch from the top of the first page on the left margin, type your name, your instructor s name, the course title, and the date in military format (9 January 2007) on separate lines, double-spaced. Set your document to double-space. Below the heading, type your title. Center the title by using the center function. Do not underline your title. Do not put quotation marks around your title. However, if your title includes an author s title, punctuate that title accordingly. Margins Except for page numbers, leave margins of one inch at the top and bottom and on both sides of the text. You may have to reset the left and right margins to.0 inches if they are preset at.25 inches. Page Set-up under File will give you the details. Indent the first word of a paragraph one-half inch from the left margin (preset on most computers). Indent set-off quotations (quotations longer than four typed lines) one inch from the left margin, but do not change the right margin. Do not use quotation marks at the beginning and end of the indented quotation. The indenting takes the place of the quotation marks. However, if dialogue is included within the indented quotation, you will need to reproduce any quotation marks in that passage. The citation for a long quotation appears after the punctuation at the end of the quotation; it is not followed by a period. Spacing The entire research paper must be double-spaced. Do not justify the lines of text at the right margin; turn off the word processor s automatic hyphenations feature. If a paper must be handwritten for a specific, approved reason, follow instructor s directions for spacing. Space one space only after end punctuation, e.g., period, question mark, or exclamation point, unless your instructor prefers the traditional two spaces after end punctuation. Page Numbers All pages will be numbered consecutively in the paper in a header that is right justified as specified above. This includes the Works Cited Page and the Author s Note, which should both be part of the same document/file to insure consecutive pagination. However, the outline is a separate document which is paginated with lower case roman numerals bottom center. Do not use the abbreviations p. or pp. or pgs. before the page number or add any punctuation mark

Davenport Community Schools June 2009: 6 URLs and Web Sources In the past, MLA recommended including URLs of Web sources in works-cited-list entries. Because of the ever changing nature of the Internet, including URLs has become ineffective. Readers are now more likely to find resources on the Web by searching for titles and authors names rather than typing URLs. You should include a URL as supplementary information only when the reader probably cannot locate the source without it or when your instructor requires it. If you do present a URL, place it immediately following the date of access, a period, and a space. Enclose the URL in angle brackets, and conclude with a period. If a URL must be divided between two lines, break it only after the double slashes or a single slash. Child, L. Maria, ed. The Freedmen s Book. Boston, 866. Google Book Search. Plagiarism Secondary Language Arts Department Procedures Davenport Community Schools Davenport, Iowa Plagiarism is the theft and use of the ideas or writing of another as one s own, or the appropriation of passages from another and use of them as one s own (American Heritage Dictionary 00). Plagiarism also applies to students who allow others to copy their work. Students are assigned writing assignments as a learning experience or as a method for them to demonstrate their understanding of their learning. Student written work is to be original. Plagiarism is not acceptable, and all participants are subject to serious consequences. Careful documentation is required on all ideas or writing that students glean from their research. This involves the use of all printed text, including the Internet. Consequences of Plagiarism: The paper(s) will receive a zero you cannot receive credit for work that is not your own. This zero will affect your quarter/ term/semester grade. If the paper is worth 00 points, you will not receive 00 points. You will be given the option to write a different paper; you must provide proof of your own work at every stage of the writing process. Presenting another s work as one s own is a serious ethical violation; it is wrong. There are consequences. If you are in doubt about the proper use of another s words or ideas in your writing, consult your teacher before submitting the paper. Please sign below and return the signed portion to your Language Arts teacher. I have read the District s Secondary Language Arts Plagiarism Procedures, and I understand the consequences of plagiarism. NOTE TAKING The best way to avoid plagiarism is to take careful notes. When taking notes, always do the following: First, read the entire text and summarize it in your own words. Then paraphrase important points and copy usable quotes. Three significant words or more need to be enclosed in quotation marks, whether or not they are actually quoted in the text. Carefully distinguish between material that is quoted, material that is paraphrased, material that is summarized, and your own words and ideas. Consider using different colored ink for each type of source or label the type of note taken, e.g., quotation, paraphrase, or summary. Include in your notes all the information you will need to cite your sources and follow the note taking as directed by the teacher. Copy all source information into your working bibliography using the MLA format. Print any Web pages you use. While online and looking at the site, write the URL and the date on the Web page if it isn t included on the printout, and record the source documentation needed for the MLA Works Cited page. Searching backwards to find the home page may be necessary. Save all your notes and printouts until you receive your final grade. CITING SOURCES You must cite the source of every quotation, every paraphrased passage, and every summarized idea you use in a research paper. Commonly known facts, such as dates, definitions, or common biographic data do not need to be cited. If you take specific, unusual, contradictory, or one-source facts directly from a specific reference source, such as an encyclopedia, you will need to cite the source. If you re not sure whether a source should be cited, include a citation, just in case. Sources must be cited BOTH in the body of the paper as an in-text citation and on the Works Cited page. In the body of the paper, you must do the following: Copy quoted material exactly, enclose it in quotations marks, and name the author immediately before or after the quote. Use the same procedure for summarized or paraphrased material, but omit the quotation marks. Cite the source information (author and page) for the quotation or paraphrased or summarized information in parentheses within the text. List on a Works Cited page at the end of your paper the information for all the sources you have cited. These sources need to be alphabetized, appear in correct MLA form, and be double-spaced as shown on the Sample Works Cited page. The Works Cited is a list of all the sources you used both those you cited and those you used for research, but did not cite directly. The Works Cited page should follow the MLA format. WRITING THE PAPER The following tips on the writing process also will help you avoid plagiarism. Read your notes carefully and make sure you understand the material before you begin to write. Write an outline and/or preliminary draft without looking at your notes. In your draft, leave spaces where you think you ll want to include quotes or supporting material. Use your own words as much as possible. No one expects you to write like an expert or a professional writer. You should, however, write like a serious, intelligent student. Cite all sources AS you write your rough draft. Trying to reconstruct your sources after the fact is extremely time consuming. Do it correctly from the start, and you won t have to start from scratch when you think you re already finished. Read through your final draft and make sure all uncited ideas are your own by Education World. Education World grants educators permission to reproduce this page for classroom use. STUDENT RECORDS ACCESS The Davenport Community School District maintains records on each student. These records contain information about the student and his/her education and may include but are not limited to the following types of records: Identification data, attendance data, record of achievement, family background, aptitude and achievement test results, educational and vocational plans, honors and activities, discipline reports, objective counselor or teacher observations, and external agency reports. Parents of handicapped students, or handicapped students over age 8, will be informed when personally identifiable information in the records is no longer needed to provide educational services, and before the information is destroyed. The records of each student are generally located in the school, which the student is attending. Parents of students under age 8 and students over age 8 (eligible student) may exercise the opportunity to review educational records of the student. Copies of student records will be made available to parents and eligible students at a reasonable cost. The fee shall be waived if it would keep the parent or student from viewing the records. Requests from parents or eligible students to view student records will be honored without unnecessary delay and in no instance more than 45 days after the request is made

69 DISTRICT POLICIES Cell Phones and Electronic Devices The Board recognizes the potential need for our students to communicate with their families under certain circumstances while they are attending school. The District offers the use of school phones as the first means of communication if the need arises during and after school hours. During the instructional day, students may not use cell phones or other electronic devices unless given written permission from the principal/assistant principal. Under no circumstances are cell phones or other electronic devices to be used to take photos or audio/video recordings of students in locker rooms, restrooms, or other private situations at school or at District sponsored activities. It is the responsibility of the Superintendent or superintendent s designee, in conjunction with the building principal, to develop a standard administrative regulation for this policy to provide consistency throughout the District for students. Phones are to be turned off and out of sight during the instructional day. If a devices has been confiscated the parents/ guardians will be notified as such and are required to pick up the device from the school. While the primary responsibility for appearance rests with students and their parents/guardians, the administration reserves the right to judge what is appropriate and what is not. DISCIPLINE Davenport Community School Districts discipline policy is an attempt to change inappropriate behavior. In general, it has been found that the parents/guardians of Davenport Community School District have been supportive in the discipline measures taken to modify their child s behavior. The different climates and curriculum presented in each class offer diverse learning opportunities to meet the needs of each student. When guest teachers are in the building, exemplary conduct is expected of students. Any student sent to the office as the result of a behavioral referral from a guest teacher will be disciplined. All teachers of the Davenport Community Schools have a uniform policy to handle discipline in their classrooms. There are, however, general guidelines that will be used for classroom management: ) All classroom teachers will conduct instruction in a positive environment. Examples of prohibited conduct include name-calling, using derogatory slurs, causing psychological harm, threatening or causing physical harm, or wearing or possessing items depicting or implying hatred or prejudice. For the Harassment Policy see // Students who believe they have been the subject of harassment or have witnessed harassment are encouraged to discuss the matter with the Building Principal; Assistant Building Principal; or DCSD Equity Coordinator, 606 Brady Street, Davenport, IA or by phone at for appropriate action. Complaints will be kept confidential to the extent possible given the need to investigate. An investigation of the complaint will be undertaken promptly. Photo Release and Media Contact As a public entity, the Davenport Community School District promotes an open-door philosophy that includes welcoming the news media to visit schools. News reporters and photographers frequently are invited to cover student and staff achievement as well as innovative programs in our schools. This is done so the community will better understand and support our important mission of education. The school administrator or a designee supervises all media visits to the school, ensuring they are conducted in a way that does not disrupt nor detract from classroom learning and student welfare. The best interests of students always are a top priority during such visits. Administrators will assist media representatives in choosing students to interview or photograph/videotape, being sensitive to obtain at least verbal parent/guardian permission if there is any question in mind that the parent/guardian may or may not approve of their student participating. To assist in this process, parents/guardians who do not want their students to be interviewed or photographed/videotaped at anytime or for any reason at school are encouraged to contact the school office at (list phone number here) so this fact can be recorded. Section 504 of the Rehabilitation Act of 973 Section 504 of the Rehabilitation Act of 973 prohibits discrimination against person with a disability in any program receiving federal financial assistance. In order to fulfill obligations under Section 504, the Davenport Community School District has the responsibility to avoid discrimination in policies and practices regarding its personnel and students. No discrimination against any person with a disability will knowingly be permitted in any of the programs, practices or activities sponsored by the school. The Davenport Community School District has responsibilities under Section 504, which included the obligation to identify, evaluate, and if the student is determined to be eligible under Section 504, to afford access to appropriate educational services. If the parent/guardian disagrees with the determination made by the professional staff of the school district, s/he has the right to a hearing. Student Abuse The Iowa Department of Education has established rules providing for the reporting of suspected physical or sexual abuse of students by school employees. District employees are mandatory reporters. They do not investigate abuse allegations. If there is a suspicion of abuse, District employees are required to report the incident. If you wish to report the alleged abuse of a Davenport student by a school employee, please contact the school principal or the alternative investigator. Unsafe School Option Under the federal No Child Left Behind Act of 200, section 9532, each state receiving federal funds is required to establish and implement a statewide policy requiring that a student attending a persistently dangerous public elementary or secondary school or who becomes a victim of a violent criminal offense while in or on the school grounds of a public elementary or secondary public school that the student attends, will be allowed to attend a safe school within the district. Inquiries regarding this policy can be made to: Ethel Reynolds, Executive Director, Davenport Community School District, 606 Brady Street, Davenport, IA Telephone: (563) BULLYING PREVENTION RESOURCES The District is committed to providing all students with a safe and civil school environment in which all members of the school community are treated with dignity and respect. To that end, the District has in place policies, procedures, and practices designed to reduce and eliminate bullying as well as processes and procedures to deal with such incidents when they occur. Following are the rules for students to follow:. We will not bully others. 2. We will try to help students who are bullied. 3. We will try to include students who are left out. 4. If we know that somebody is being bullied, we will tell an adult at school and an adult at home. The following are web-based and local resources for parents/guardians and their students to learn more about preventing bullying and how to handle bullying should it be an issue for your child

Homeless students must be given access to the same public education, including preschool education, provided to other children. The goal of the homeless program is to ensure every student, regardless of their living situation, is prepared to come to school every day and learn. The main focus of the McKinney Vento Act is to make sure a homeless students education is not interrupted due to moving, lack of transportation, or other barriers to their education. I understand that my success in all Davenport Community Schools depends upon me following these policies. I also understand that violation of these rules can lead to ISS, OSS, Disciplinary Hearings, Expulsions, and exclusion from after school activities, including athletic events and dances. I also understand that my attendance and grades are important indicators of my success in school. I will make my best effort to satisfy the requirements set forth by the district; otherwise, I expect to have appropriate consequences for my actions. (Student signature) (Date) 42 43
