1 Milton Band Marching Band/Colorguard Registration Form STUDENT: Fall 2017 Grade Participation Fee: This fee provides less than one-half of the annual funding needed by our band programs the rest will be obtained by fundraising activities. These funds are used to help pay for items such as band and ensemble props, equipment transportation, equipment repair/supplies, accessories, guest clinicians, judging fees, additional instructors, music for bands and ensembles, show design, equipment purchases, band office operations and other miscellaneous expenses related to the entire MHS Band Program. Trip expenses (other than football games) are not included in this fee and will be paid for separately by those who participate. Fulton County provides funding for only a portion of the costs of maintaining a music program with the level of activity, quality and the opportunities enjoyed by the members of the MHS Band Program. Participation Fee is not tax deductible. $250 for 1 st student, $225 for 2 nd in same family, $200 each for 3 rd or more $ (Annual fee for participation in the marching and concert band program.) I/We agree to attend all required practices, camps, performances, and will pay all fees associated with the marching band. We understand that because marching band is an extra-curricular activity, student membership in the band is left to the discretion of the director. Failure to follow the guidelines of the student behavior code or other policies outlined in the band handbook or on the MHS Band website can result in disciplinary action or dismissal from marching band without a refund. Also, because of the nature of marching band expenses, we are aware that there are no refunds given for marching band fees for any reason. Should a member fall behind on payments, it could result in dismissal from the group. In consideration of permitting my child to participate in all MHS Band/Colorguard events, and all traveling associated with the organization, I hereby agree to indemnify and hold harmless Milton High School, Fulton County Schools, its teachers, employees, booster clubs and trip sponsors against any claim for damages, compensation or otherwise on the part of said minor or his heirs, executors, or administrators. The undersigned consents to the presentation and use of photographs of the above student on the Band s website and in connection with other Band and Milton High School publications and uses. Acknowledgement of Parent and Guardian of the foregoing and of receipt of information regarding costs, rules and regulations regarding participation in Marching Band: Parent s Signature: Date: Print Name: Milton Band Pledge Form The Milton High School Band could not operate its best in class program without the generous donation of time by its many volunteers and students. To enable the Band Booster Club to plan its support of the Milton Band for the coming year, the Club is asking each family to make an investment of time by committing to a minimum amount of volunteer time. This commitment may be filled in a wide variety of ways that best fits your family s schedule. The family pledges to (select one): Work at least 6 Verizon Amphitheatre Shifts* AND the recommended Volunteer Service Hours for Milton Band (40 hours per Family is suggested) - OR - Work at least 6 Verizon Amphitheatre Shifts* AND make a cash donation of $ in lieu of volunteering time for Service Hours (donation does not count toward Verizon shifts) *Each person that works a concert counts as 1 shift. For example, if you and a family member or friend, etc. work a Verizon event, this counts as two shifts towards your volunteer requirement for Verizon. NOTE: The compensation that Milton Band receives for band parents/studentsstaffing Verizon is a key revenue source for the Band s annual operating budget. As such, if the family obligation is not met, you will incur additional band fees up to a maximum of $330 ($55 for each concert missed). (Parent Signature) (Date)

4 Milton Band Marching Band Meals The Milton High School Marching Band Meal program consists of Friday night dinners before football games and meals and snacks on competition days. Our program serves meals to over 160 marching band members and staff. For many band families, this is a great convenience as alternate arrangements are not needed to feed the band students. Our Friday night dinners include a main dish, a side, and normally a dessert. The menu for the week will be announced in the band blast that is sent out every weekend. Although we are sensitive to the many dietary restrictions of our band students, we cannot ensure every meal will meet the needs of some students. While we are able to accommodate the majority of these dietary restrictions, we cannot accommodate them all. As such, if your child has a dietary restriction, please review the weekly band blast to determine the menu for that week. If you have concerns, please evaluate the restaurant's menu for the band meal to be provided that week. If it does not meet your child s dietary restrictions, we ask that you please plan accordingly with your child and provide an alternate meal. If you have any questions, feel free to contact Kari Devlin or Kristin Kepner. 5 Milton Band Parent Shirt This is an optional item that can be worn to any band event, such as when volunteering, at football games, and at marching band competitions. The shirt runs true to size, will not shrink, and comes complete with your student s name and instrument or ensemble embroidered on the sleeve.
