1 2017 HOGS 7/8 Team Camp At the University of Arkansas Coaches who are interested in participating in team camp must return the completed registration form (one per attending team). Registrations spaces will be filled on a first come, first serve basis. Seeding will be done by the order they are received. HOGS HS Team Camp Cost *per athlete (3-day camp) (The NCAA recognizes 9-12 graders as recruitable student athletes. Team Camp Refund Policy Acknowledgement Form This form lets us know that you have read and understand the deadlines associated with attending Arkansas Volleyhogs Team Camp Teams are accepted in the order in which the Team Registration form is received (either by time/date stamp on the or postmark on the envelope) Make sure that you fill out one form per team an indicate Varsity, JV, SOPH, FROSH if applicable. First $250 received to Arkansas Volleyhogs will secure your place in 7/8 Team Camp. Because each school handles money differently, we will accept a combination of PO, Booster check, District Check, or a combined total from your athletes that register online. Hope that makes sense. If we have not received the minimum deposit by May 31, 2017 we will drop your team from camp. The registration form does not need to have your complete roster if you are unsure of who is attending. 3 Fayetteville, AR Seeding Tournament seeding will be determined according to the date we receive your Team Registration Form and minimum deposit ($240). A confirmation will be sent to you when your completed application is received and processed. T-Shirts Only Teams submitting rosters and payment by June 1, 2017 will receive an accurate T-Shirts order. After June 1, your order will be filled as best as possible with no guarantee sizes will be accurate. Meals & Housing Overnight Athletes/Coaching Staff will be provided the following meals in the dining hall: Day Two Breakfast, Lunch (boxed lunches to go) Commuter Athletes will be provided the following meals in the dining hall: Day Two Lunch (boxed lunches to go) Additional Commuter Coaching Staff have the option to purchase a meal ticket for $40 to cover the cost of meals: Day Two - Lunch (boxed lunches to go) Tickets may be purchased either over the phone with Larissa or on the day of. Please let Larissa know by July 1 how many tickets your coaching staff intends to purchase so that we may have an accurate count for dining services. Overnight Camp will be housed in Pomfret Droms. We have heard that Pomfret may be under renovations this summer. If we can t get in Pomfret, we will house in Humphries/Yocum. Arkansas Volleyhogs is not responsible for providing overnight supervision at any time during team camp (either on or off campus). We will have a dorm counselor staying overnight in the dorms to assist the visiting teams coaching staff. They are not babysitters for your team. If your team is staying in the dorm, a coach/s must be staying overnight as well. Requests for on-campus housing will be taken through May 31, 2017 WHAT TO BRING Overnight campers should bring t-shirts, shorts or spandex, knee pads, socks, playing shoes, personal toiletries, alarm clock and spending money for Camp Store and vending or incidentals. Camp Store will have pizza on sale nightly in the dorm and a variety of Arkansas Volleyball apparel. The dorm DOES NOT provides linens, pillows and blankets during your stay. Please pack accordingly. Arkansas Volleyhogs, Inc. and Guest Conference Housing/Dorms are not responsible for lost or stolen items. Campers are allowed to bring snacks, drinks and coolers from home; however, there are no ice machines available in the dorm.

4 Deadlines & Refund Policy HOGS 7/8 Team Camp You have Until May 31, 2017 to finalize your plans without penalty. First $250 received to Arkansas Volleyhogs will secure your place in 7/8 Team Camp. Because each school handles money differently, we will accept a combination of PO, Booster check, District Check, personal check, or a combined total from your athletes that register online. If we have not received the minimum deposit by May 31, 2017 we will drop your team from camp. Once we receive the minimum deposit, you will be notified and given a seeding number for the competitive portion of the camp tournament and will be considered a committed team. Any new team registering after June 15, 2017 will be charged a late registration fee of $10/athlete. If you were placed on the wait list due to the camp being sold out, and are offered a space due to cancellation, the late registration fee will not apply. Any committed team that drops on or after June 1, 2017 will be charged a non-refundable deposit of $30/athlete (you will be charged at least a minimum of 8 athletes per committed team or $240/team). Any committed team that drops on or after June 20, 2017 will be charged a non-refundable deposit of $50/athlete (you will be charged at least a minimum of 8 athletes per committed team or $400/team). Refund Charges do NOT apply for roster changes if another athlete is attending in the place of the athlete that has dropped. (If Sally can t attend and Jane is going in her place, Sally will receive 100% of all monies received, Jane will register in her place without penalty) However, if Sally can t attend and there is no one to take her place, Sally will receive a refund less $30 plus applicable finance charges if she withdraws on or after June 1, Sally will receive a refund less $100 plus applicable finance charges if she withdraws on or after June 20, o o Anytime on or after June 20, 2017 requests for full refunds for a single athlete attending team camp will be accepted for injuries that occur or medical conditions that would prevent your athlete from participation in camp. A signed statement from your physician must accompany the written request. Overscheduling of volleyball-related activities is NOT considered a valid medical reason, nor is fatigue and muscle soreness; and will be subject to the above deadlines and penalties. Refunds are not given to participants who (1) Withdraw after June 20, 2017 for any reason other than medical, (2) Voluntarily withdraw from camp at any time for any reason, (3) Are sent home for disciplinary reasons. NO-SHOWS are NOT entitled to any refund whatsoever. All monies should be received by Arkansas Volleyhogs by July 1, 2017, unless other arrangements have been made with Larissa Watson.
