2 CP Rochester January April 2013 Registration Form 2013 Recreation Programs This year we will continue to have the recreation brochure span a 4-month period because it will align more closely with the school year; we hope this will make it easier for families! About the People of all ages and abilities participate in CP Rochester s Community Recreation Program, from preschoolers to adults. The primary goal of our program is to provide opportunities for individuals who have a disability to develop lifetime recreation skills and interests. Although we are continuing to offer group and family-oriented outings, we encourage parents to take advantage of the respite service so that caregivers can have a break through our recreation programs. 3 Family & Group Outings Breakfast with the Easter Bunny The Easter Bunny will be visiting the Elks Lodge again this year to greet all the girls & boys, bringing a basket of goodies for each. Join us for breakfast with the Bunny! 4 The enrollment process STEP ONE: Contact your Medicaid Service Coordinator to complete an Intake Packet for our service prior to participating in any of our recreation programs. Application materials can be obtained from our website at: STEP TWO: Once you have been approved for our service, you must be waiver enrolled prior to participating in a recreation program. Once you are waiver enrolled, you will be added to our mailing list to receive our recreation brochures and be ready to register for our programs and events! How to register for programs All required intake documentation must be received and processed prior to registering for an event or program. To register for a recreation program or event, complete the registration form on the last page of this brochure and mail it in to: CP Rochester 1000 Elmwood Avenue, Suite 900 Rochester, NY Certain group outings and events are listed on our website and available for online registration. You can view these events on the CP Rochester Events calendar at Once your registration is processed, you will receive a confirmation letter confirming the date(s) and time of the activity. Please keep this as a reminder for your upcoming program(s)! The confirmation letter will tell you when payment is due. Page 2 Family & Group Outings Rochester Museum & Science Center Feature exhibit Race: Are we so different? Like a fingerprint, each person is unique. We are all an exquisite combination of our parents, grandparents, and ancestors, which gives us our one-of-a-kind look. Every person experiences life somewhat differently because of his or her appearance, too. We often refer to it as race. Explore the stories of race from the biological, cultural, and historical points of view. Discover what s not so visually apparent hint: we re not all that different! You can also enjoy the permanent exhibits of the RMSC during your visit! All Ages Program # 19 Sat, January 12 Program # 22 Sat, March 9 Program # 20 Sun, February 3 Program # 23 Sun, April 21 Time: Check in at 10:00am Deadline: 1 week prior to date Location: RMSC, 657 East Ave FREE Admission for participants & immediate family/caregivers IMAX Show Coral Reef Adventure At the Strasenburgh Planetarium Check out this marine adventure an octopus hides in plain sight among the corals, perfectly camouflaged, while a lionfish waves its feathery, but poisonous, spines. Suddenly, a 300-pound potato cod, feeling threatened, changes its spots right before your eyes! The IMAX movies are appropriate for ages 6 and up and last approx. 50 minutes. Admission to the museum is free after the movie. For all shows: FREE admission for participants and immediate family/caregivers Deadline: 1 week prior to date Location: Strasenburgh Planetarium, 657 East Ave, Rochester Program # 21 Date: Sat, January 12 Time: 2:30pm Program # 22 Date: Sun, February 3 Time: 1:00pm We will offer a group outing to IMAX on Sat, Mar 9 and Sun, Apr 21 Shows & times TBA Page 11
5 Gymnastics & In-Line Skating in conjunction with SUNY Brockport Program # 17 Gymnastics Jump, run, flip, and be creative in this gymnastics class at SUNY Brockport that is supported by members of the Gymnastics Team and students in the Physical Education program. Participants are kept moving from station to station and are matched up with a volunteer throughout the class. Try out the balance beam, roll and flip on the mats, hop over the horse, bounce on the trampolines, and everyone s favorite, hop into the big foam pit to relax at the end of class! Approx 6 classes. Ages: 6-13 Class length: 45 minutes Cost: FREE Group Size: Limit 8 kids Location: SUNY Brockport, Tuttle Building Gymnastics room Program # 18 Skating This spring we are exploring the sport of skating. The Brockport Graduate Assistant leading this program is hoping to combine opportunities for in-line skating (rollerskating) and ice skating with support from the Physical Education students. In-line skates and all the safety pads (for elbows, knees) & helmet are provided. Ice-skating equipment may require a small additional rental cost. TBA. Approx 4 classes. Ages: 6-16 Class length: 1 hour Cost: FREE Group Size: Limit 8 kids Location: SUNY Brockport Campus (outdoors) PLEASE NOTE Dates & Time TBA for Gymnastics & Skating: Timeframe is likely to be a Feb start for gymnastics and March for skating. If you are interested in either of the above programs, please indicate so by indicating the appropriate program number on the registration form and we will know to follow up with specific details. Program Fees / Payments & Cancellations Program Fees We try our best to offer low or no-cost recreation programs. However, we do incur expenses beyond basic staffing and this is why there are fees listed for many activities. If there is a cost listed for the program, that is the fee that must be paid in order to participate. This cost applies to all participants. For respite programs (), you must be enrolled in the Home and Community Based Services waiver in order to participate. If you are enrolled in the HCBS Waiver, you still have to pay the fee that is listed for that activity. The same applies to individuals who are enrolled in the Care At Home Waiver. We will provide a receipt for payments made to CP Rochester for Recreation Programs. You may choose to submit your receipt for Family Reimbursement please contact your Medicaid Service Coordinator. Payments or Cancellations Payments or cancellations must be made by the program s registration deadline. If we have not received payment one week prior to the event s date or start of the session and you have not contacted us, we will assume that you are not going to participate in this program anymore. Past due payments will affect registration for future recreation programs. In the event that a program is cancelled, we will issue a voucher that can be used toward future recreation programs; we do not issue refunds. 6 Aquatics Individual Swim Lessons Weekdays Instruction is provided by our Adapted Physical Education teacher and swimming instructors and is designed for individuals who have special needs. Students will receive 30 minutes of instruction and will work toward developing independent swimming skills and increased safety awareness in the water. Space is limited!! Please indicate your preferred time on your registration form and include a second choice. Al Sigl Pool Lesson Times: 4:30pm, 5:00pm, 5:30pm, or 6:00pm Location: Al Sigl Pool, 1000 Elmwood Avenue, Door 1 Program # 1 Dates: January 8,15,22,29, February 5,12 Make-up Date: Feb 26 Deadline: December 30 Program #2 Dates: March 5,12,19,26, April 9,16 Make-up Date: April 23 Deadline: February 15 CP Rochester Pool Lesson Times: 4:00pm, 4:30pm, 5:00pm, 5:30pm Location: CP Rochester Pool, 3399 Winton Road S, Program # 3 Dates: January 11,18,25, February 1,8,15 Make-up Date: March 1 Deadline: December 30 Program # 4 Dates: March 8,15,22,29, April 12,19 Make-up Date: April 26 Deadline: February 15 For each session the cost is: $65 per person Ages: 3 and up Recreational Horseback Riding Our horseback riding program will be offered once again in the spring, likely beginning in April as the weather warms up. Our dates are not set at this time; however if you are interested in more information about riding with us next year, please indicate so by putting... Program # 16...on your registration form so that we can follow up with more details! We are exploring other riding stables as well in Monroe County that are centrally located and can still serve our students in a small-group lesson format. Thank you! Our swim lessons take place in a heated therapy pool, with the temperature averaging 92 degrees. It s a great place for young and new swimmers to learn how to swim! Maximum depth is 4.5 feet. 7 Participants who attend a Recreation Program independently must be enrolled in Medicaid s HCBS Waiver. You are welcome to attend programs that are listed in the Family & Group Outings section as well; we will provide staffing if there is interest! Please call us if you have any questions ext 2282 Dining Out for Young Adults Get together with friends or come out and meet someone new! This program is for young adults who are looking to get out into their community and enjoy a fun evening out to dinner. Program # 14 Ages: 18 and up Date: Saturday, April 20 Time: 5:30pm Cost: bring money for dinner! (approx $15) Deadline: April 5 Location: Ruby Tuesday s, 2525 West Ridge Rd, Rochester (Greece) Create your own ceramic art At Rochester ClayWorks This program is for people who would like to do something creative, fun and relaxing at the ceramics studio. On the first Saturday, you will create your own work of art in pottery, and the following Saturday you will glaze your piece. The studio is wheelchair accessible. Program # 15 Ages: 12 and up Dates: Saturday, March 23 & 30 Time: 1:00-2:30pm Cost: $25 per person Deadline: March 1st Location: 203 Milburn Ave, Rochester Page 8 Aquatics NEW! During family swim you can practice some of the skills you ve learned, have fun & relax in the warm water, or burn off some energy! Staff is present to lifeguard children must have an adult in the pool. 8 Snowshoeing at GCVM Nature Center Enjoy a traditional winter activity & the snowy weather and come snowshoe the trails at the Genesee Country Village & Museum s Nature Center! GCVM staff will provide a quick orientation to the trails and sights & sounds that we may see along the way, then off we go into the woods to discover & explore the winter wonderland. Make sure you dress in layers and warm boots! Program # 9 Ages: 5 and up Date: Sunday, February 17 Time: 1:00pm Location: Genesee Country Village & Museum Nature Center 1410 Flint Hill Rd, Mumford Deadline: February 1 Cost: $5 adults / $4 youth under 17 Snowshoe rental: $5 additional per person Group Size: Minimum 4 people Tops Cooking The instructors at the Tops Cooking School do a wonderful job of putting together creative menus and teaching participants how to make the items. At the end of the class, you will leave with the menus and directions for all your creations and can make them at home! If you are interested in more information about Tops classes this winter/ spring, please sign up for this program on the registration form and we will follow up with you when the January-March Tops Cooking School calendar becomes available! Staff are present to support our participants as part of Tops s regularly scheduled class. Program # 10 Ages: 8 and up Location: Tops Cooking School, 3507 Mt Read Blvd, Cost: $15 per person Day: Friday, dates TBA Time: 6-8pm Bowling League This winter we are offering the bowling league at AMF Fairview Lanes! Whether you enjoy bowling for fun or for competition, join us out for our bowling league! We will meet 10 times and finish off the league with a pizza party & award ceremony to celebrate. You will be grouped into teams and scores will be kept each week. Bumpers & bowling ramps are available! Program # 11 Ages: 8 and up Dates: Wed, January 16,30, February 6,13,20, 27, March 6,13,20,27 Time: 6:00-8:00pm Cost: $80 Deadline: January 4 Group Size: Limit 25 people Location: AMF Fairview Lanes, 1407 Fairport Rd, Bowling Socials Get together with your friends or come and meet some new people at the bowling alley in Fairport! The lanes are wheelchair accessible and we can have the bumpers up and bowling ramps if needed. A fun chance to get out but stay warm during the colder months! Includes 2 games and bowling shoes.
