1 All-State Tournament: First, let us congratulate you on your selection to the 2017 All State Team. We have selected the top players from four regions of our state. Coaches and scouts from across the state have selected you as a premier player in your area. You should be very proud of this accomplishment. To make this weekend a tremendous experience for everyone, we have put together an itinerary for everyone involved. If you have additional questions please refer them to your area coaches or coordinators. All players will pay a participation fee of $250 for the tournament. Please make the check payable to the. The fee will cover the banquet, all state plaque, player packages and the operating cost of the games. Payment and waiver must be submitted on the day of the All State event to participate. All games will be played at Georgia College in Milledgeville, Ga. Georgia College 231 W. Hancock St. Milledgeville, Ga Please find enclosed: Itinerary Schedule of Events Release of Liability List of Coaches and Coordinators
2 Hotel information Players are responsible for making their own travel accommodations for the tournament. We encourage parents to be present for the games, banquet and any down time that may accompany the tournament. Banquet - We will have a banquet at 7:00pm on Friday on the grounds of Georgia College. All players are covered for the banquet with their participation fee. Any guests of the players are asked to pay $20.00 for the banquet. Dress for the banquet is semi-casual and players are asked to wear khaki shorts and a collared shirt. Report times and player packages - All Northwest and Southwest players should report to Georgia College at 11:00 am Friday. All coaches and coordinators will be present to organize plans and present player packages. All Northeast and Southeast players should report to Georgia College at 1:30. All coaches and coordinators will be present to organize plans and present player packages.

4 RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT BY SIGNING THIS DOCUMENT, YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. PLEASE READ CAREFULLY! In consideration of allowing me (and my child) to participate in the Tournament from 6/01/17 through 6/05/17, and for other good and valuable consideration, the receipt and sufficiency of which is acknowledged, I hereby agree as follows: 1. I waive any and all claims that I have or may have in the future against the Georgia Dugout Club and their members, directors, officers, agents, employees, assigns, independent contractors, representatives and successors (hereinafter collectively referred to as the Releases ) and to release the Releases, both individually and collectively from any and all liability for any loss, damage, expense or injury, including death, that I or my next of kin may suffer as a result of my participation in this Tournament sponsored and coordinated by the ; 2. I hold harmless and indemnify the Releases from any and all liability as to any claims for contribution or indemnification asserted by a third party resulting from my participation in the Tournament. Specifically, but without limitation, should I sue a third party, and should such a third party bring a Third Party Complaint against the or other Releases, then I will hold harmless and indemnify the Releases in defending such third party actions; 3. This agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns and representatives in the event of my death or incapacity; and 4. This agreement and any and all rights, duties and obligations as between the parties to this agreement shall be governed by and interpreted solely in accordance with the laws of the State of Georgia.
5 In entering this agreement, I am not relying on any oral or written representations or statements made by the Releases with respect to the Tournament, other than what is set forth in this agreement. I confirm that I have read and understand this agreement prior to signing it, and I am aware that my signing this agreement, I am waiving certain legal rights which I or my heirs, next of kin, executors, administrators, assigns and representatives may have against Releases.
